JOB SUMMARY:

The Home Hemodialysis Unit is engaged in caring for patients who have selected home hemodialysis as a renal replacement therapy. The successful applicant will work in collaboration with the Nephrologists, the Allied Health team and Nursing team to support the coordination of education, training and monitoring of patient. The successful candidate will incorporate the attitude, values, knowledge and skills required to support clinical needs and independence; introducing the concept of self-management, self-care and promoting a culture of "Home".

Qualifications:
  • Current Registration in good standing with the College of Nurses of Ontario
  • BScN preferred
  • Applicants with current Canadian Nurses Association certification in Nephrology Nursing will be given first consideration. Applicants currently enrolled or willing to enroll may be considered and will be required to provide proof of completion within two (2) years of starting in the position
  • Completion of Nephrology courses/certification; BCLS
  • Minimum of 3 years current Hemodialysis experience required
  • Candidates with Home Hemodialysis experience will be given first consideration
  • Skills in assessing, planning, implementing and evaluating the ongoing clinical management of Dialysis Therapy
  • Demonstrated knowledge of nephrology theory & clinical renal nursing practice
  • Demonstrated experience, ability & knowledge in the principles of adult teaching
  • Demonstrated ability to work autonomously and within team environment
  • Computer skills and Nephrocare/Cerner experience preferred
  • Demonstrated good interpersonal and communication skills with an ability to work with a multidisciplinary team, including other health team members, patients, family members and visitors
  • Demonstrated expertise in management of health problems and support needs for home hemodialysis patients in the home setting.
  • Ability to be flexible and effectively work in a busy environment with changing priorities and tight deadlines
  • Ability to prioritize & complete assignments in a reasonable length of time
  • Demonstrated sound judgment, good observation and patient assessment skills, tack and empathy
  • Demonstrated evidence of recent and relevant continuing education considered an asset
  • Demonstrated understanding of and commitment to Grand River's comprehensive safety programs and practices, including staff, patient, and environmental safety, and participation in regular in-service education in this area
  • Must have own reliable vehicle and valid driver's license for home visits.
  • Demonstrated good attendance record.
