Responsible for providing clinical and quality support, guidance and education to the residence's nursing staff, residents and their family members in order to ensure the continuing care needs of our residents are met. Assists in maintaining a safe and secure environment for residents, visitors and other staff members. All duties are performed in the best interest of the residents and their families, and in accordance with Chartwell's vision, mission and value statements.

The responsibilities of the role include but are not limited to:
  • Ensures all nursing staff are able to meet resident care needs by providing and overseeing clinical education and training for Health Care Aides in topics such as palliative care, pain management, abuse prevention and awareness, etc.
  • Monitors Health Care Aide activities to ensure that resident care standards are met and maintained in accordance with current policies and procedures.
  • Responds to resident or family member concerns and ensures appropriate action is taken within decision-making authority and/or brings to the attention of the Director of Care.
  • Collaborates with the Director of Care to evaluate nursing staff competencies and identity required education/training requirements.
  • Researches/identifies internal/external programs to meet education/training requirements.
  • Schedules, co-ordinates and facilitates/conducts in-service education programs to meet identified needs.
  • Provides leadership and direction to nursing staff in the provision of continuing care in the role of clinical nursing support.
  • Ensures that resident care is consistent by constantly evaluating Health Care Aide work assignments and routines, residents care plans/programs.
  • Leads and co-ordinates the Continuous Quality Improvement process.
  • Liaises with nursing department, committees and multi-disciplinary departments to advance goals and quality improvement plans.
  • Co-ordinates and communicates within the nursing department and external health care providers to ensure care plans are being followed and updated as required.
  • Networks and liaises with Directors of Care, educators and other health care professionals.
  • Participates on the Quality Improvement Committee to develop and implement best practices throughout the organization.
  • Develops benchmarks and indicators to ensure care standards are met and completes required audits.
  • Provides support with investigations and makes recommendations on appropriate corrective action where a serious incident has occurred.
  • Ensures that nursing staff is current in competencies, assessments, licensing, certification and other educational/training requirements.
  • Conducts duties in accordance with Chartwell policies and procedures, as well as, government regulations, laws and policies.
  • Understands and follows all health and safety policies and procedures. Works safely to reduce the risk of injury to self, other staff members and residents. Promptly reports all actual or potentially hazardous situations.
  • Maintains confidentiality of residents' personal information.

The ideal candidates will possess:
  • Minimum of 3 to 5 years of relevant work experience in an Assisted Living or Long Term Care community is preferred.
  • Prior experience working with seniors.
  • Must have clinical education experience
  • Post secondary education in an accredited nursing program and current nursing registration as an LPN or RN in the province of employment
  • Good organizational and time management skills with the ability to multi-task.
  • Ability to develop and deliver clinical training.
  • Knowledge of the Continuing Care Health Service Standards (for supportive living) and policies and procedures for the province of employment.
  • Must possess excellent communication (written and verbal) and interpersonal skills with the ability to exercise tact and diplomacy in dealing with residents and other staff members.
  • Excellent conflict resolution skills.
  • Computer literacy in a Windows environment including email, word, power point and excel.
  • Must be available to work the hours necessary to ensure the effective operation of the residence.
  • Ability to respond to emergency situations (as per Chartwell policies and procedures).
  • Criminal Records Check and Vulnerable Persons Check.
