Summary:

The successful candidate's primary focus will support early management of Chronic Kidney Disease (CKD) which includes CKD identification, prevention, and the concepts of patient self-care management.'

Responsibilities:

Responsibilities also include patient screening, preparation and promotion of independent modalities. These responsibilities will be in collaboration with the Nephrologists, Allied team and the CKD clinic nurses; within a mentorship environment. From time to time the candidate may be required to work at the KW Renal Clinic. Contributes to ensuring there is a safe environment for patients, staff and visitors.'

Other information:

Current registration (in good standing) with College of Nurses of Ontario

3 years (full time equivalent); Acute nursing experience preferred.

BCLS

Current relevant Nephrology/ CKD experience preferred

Completion of health assessment course preferred

Demonstrated knowledge and skill in the areas of patient adult education, health promotion, CKD and Chronic Disease management.

Demonstrated effective communication skills necessary to interact with patients, their families, the multidisciplinary team, CCAC and other health care facilities.

Demonstrated knowledge of General Medicine/Nephrology, Peritoneal Dialysis, Hemodialysis, Diabetes education and Hypertension.

Skilled in BG testing, 12 lead EKG's, vital sign monitoring, IV therapy, aseptic technique and oxygen therapy.

Responsible, energetic individual with above average ability to prioritize and organize in a busy environment

Demonstrated ability to work effectively within a team environment

Ability to respond to emergency situations by rapidly identifying priorities and complex patient needs

Demonstrated evidence of recent continuing education

Good attendance record

Basic computer skills

Valid Driver's License
