Summary:

The Oncology Clinic Nurse, as a member of the interdisciplinary team, participates collaboratively in the assessment, development and implementation of the plan of care for patients. As the key contact person and navigator for patients, she/he supports the patient throughout their cancer journey, facilitating referrals and linkages when they need them both within the cancer center, GRH and the patient's home community. The Clinic Nurse participates in the development, implementation and ongoing evaluation of the activities within the Outpatient Oncology program and participates and/or supports both education and research activities. He/She also provides telephone assessment and consultation to patients. Contributes to ensuring there is a safe environment for patients, staff and visitors.'

Other information:

Registration with the College of Nurses of Ontario

Three to five years recent experience in Oncology nursing

deSouza Foundations of Oncology Nursing Practice or equivalent oncology certification required

deSouza Nurse Navigation recommended

Canadian Nurses Association certification as a Certified Oncology Nurse (CONC) or commitment to complete CONC in a timely manner

Demonstrated experience in working within an inter-disciplinary team

Working with a variety of computer systems and software applications

Participation in education and clinical training programs

Experience in telephone nursing support

Good attendance record

Demonstrated knowledge and understanding of:

Oncologic disease processes

Patient assessment

Principles of adult learning

Needs of the oncology patient population and patient focused care

Case coordination within the Cancer Centre, GRH and community agencies

Empathy with patients and family members

Therapeutic communication techniques

Principles of continuous quality improvement

Critical thinking and problem solving

Ability to work independently as well as within an interdisciplinary team
