The Regional Municipality of Durham's Long Term Care Division is currently seeking temporary full-time Nurse Practitioner and Health Services Managers.

Reporting to the Administrators, the Nurse Practitioner & Health Services Managers will be responsible for collaborating with the Medical Directors, other physicians and community services and agencies in the provision of primary health care for residents residing within the Region of Durham's Long Term Care Homes, which include Fairview Lodge in Whitby, Hillsdale Estates and Hillsdale Terraces in Oshawa and Lakeview Manor in Beaverton.

The incumbents will:

- Practice autonomously, offering the full scope of primary health care NP practice to ensure comprehensive assessment, analysis, treatment planning and evaluation of residents' care

- Collaborate with the Director of Care in the provision and advancement of nursing services and the development of best practices - Act as an educator for administration, managers, staff, residents, families and the community

- Assist with the day-to-day human resources of direct reports and departmental staff, which includes hiring, disciplining, training and coaching staff to ensure optimal performance

- Effectively plan, both strategically and operationally, to address the changing and increasingly complex care needs of the residents

- Effectively communicate with interdisciplinary team members including nursing staff, medical staff, allied health professionals, families, residents and professional colleagues

- Work in accordance with the provisions of applicable health and safety legislation and all corporate/divisional and home (s) specific policies and procedures

- Act as a resource for administration, managers, staff, residents, families and the community

- Act as an advocate through promotion of evidence-based primary health care services and nurse care of older adults

The successful applicants will possess:

- Baccalaureate in Nursing and proof of current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class; Master's Degree in Nursing is preferred

- Minimum of 5 years' experience in gerontological nursing and/or as a primary health care nurse practitioner, preferred

- Successful completion of the Ontario Primary HealthCare Nurse Practitioner Certificate Program

- Current Membership in the Nurse Practitioners Association of Ontario and the Registered Nurses Association of Ontario (RNAO)

- Knowledge of the Standards of the College of Nurses of Ontario, LTC Standards, Pharmacy Standards and the Regulated Health Professionals Act - Knowledge of Infection Control and Risk Management

- Current Canadian Gerontological Nursing certification or current plans to obtain certification

- Proven leadership abilities with excellent oral, written and interpersonal skills

- Demonstrated fit with critical behaviours of core and management/leadership competencies

- A valid Ontario Class G driver's licence and access to a reliable vehicle

Conditions of Employment

The successful candidate will be required to provide a satisfactory Police Vulnerable Sector Check dated within two (2) months from date of hire. Additionally, tuberculosis screening will be required in accordance with the LTCH Act (O.Reg. 79/10). Proof of education, qualifications and any other job bona fide requirements will also be collected. In addition, all new employees working in Long Term Care must possess a minimum of 2 doses of the Covid-19 vaccine and the last dose received must be within 6 months of date of hire; otherwise, a/another booster is required. Alternatively, a valid medical exemption, as defined by the Ministry of Long Term Care, is required as a condition of hire.
