Summary:

This is a Composite Position: Community Day Services and Adult Transition Team.

Offering person-centered care, guided by recovery principles, the Registered Nurse promotes healing and instills hope while demonstrating the values of the Mental Health and Addictions Program.

The RN provides psychiatric and mental health nursing care working within a team working with individuals experiencing psychiatric and mental health problems. The patient population includes people living with a severe and persistent mental illness. Nursing care is provided in accordance with the Canadian Standards of Psychiatric and Mental Health Nursing (CNA). This position requires that the RN work as a member of an intra-professional clinical team. The RN will work primarily Monday-Friday, with flexibility required for occasional evening and weekend work to meet patient and family needs.

The SMH Day Hospital Services work with people in the community assisting them to maintain community tenure and where possible preventing hospital admissions. This dynamic service includes a number of clinics providing short and long term community supports. The program will provide a number of clinics including injection, clozapine, metabolic, dual diagnosis, as well as be a resource for community partners to better assist clients in their path to recovery.

The Adult Transition Team works with patients from both Specialized and Adult Inpatient units who require immediate community stabilization to help ensure a successful discharge. All patients coming to this team will be referred to a community service however that service may not be available upon discharge date. The role ranges from outpatient follow up, intensive case management and ACT -Like services, the average length of stay varies from one month to two years. The multidisciplinary team includes this nurse as well as an Occupational Therapist, a Social Worker and Psychiatrist.

Qualifications :

  • Current registration with College of Nurses of Ontario
  • Certification (CNA), Psychiatric and Mental Health Nursing Certification preferred, or willingness to obtain
  • Minimum 3 years' experience in Mental Health and Addictions Nursing
  • Post Diploma preparation, relevant to Psychiatric and Mental Health Nursing preferred
  • Advance de-escalation, code white training
  • Basic rescuer CPR Working knowledge of psychiatric and mental health disorders and related psychiatric/psychosocial interventions and treatment models
  • A understanding of the philosophy and principles of recovery
  • Comprehensive knowledge and experience in administration and/or monitoring of psychotropic medications
  • Excellent interviewing skills (individual/group), verbal and written communication skills
  • Demonstrated group facilitation skills
  • String skills in biopsychosocial assessments, care planning, crisis plans, psychiatric nursing interventions and complete outcome evaluation for individual care
  • Strong consultation skills with Physicians, Community Therapists, Support Workers and Families
  • Ability to work independently with consultation from multidisciplinary team members
  • Experience of working in a community mental health setting preferred
  • Demonstrated ability to work independently and resolve issues with minimum supervision
  • Working knowledge of current best practices related to treatment of Severely Mentally Ill persons
  • Demonstrated skills in crisis assessment, intervention, and de-escalation
  • Proven ability to work collaboratively with a multidisciplinary team and community service providers
  • Demonstrated ability to provide in depth mental health assessments, risk assessments, mental status examinations, and development of crisis, and discharge plans
  • Case Management experience preferred, with ability to develop strong community agencies relationships
  • Knowledge of community resources preferred
  • Working knowledge of the Mental Health Act and Community Treatment Order Legislation, Privacy Act, Health Care Consent Act, and other relevant legislation
  • Demonstrates excellent interpersonal skills, including ability to engage in conflict resolution
  • Excellent critical thinking, organizational and problem solving skills
  • Demonstrated time management and organizational skills
  • Valid Ontario Driver's License and access to a vehicle
  • Good attendance record
