Summary:

The Hazelglen program is designed to help people with urgent mental health problems avoid the need for hospitalization by providing timely mental health treatment and support in community settings and people's homes. You will facilitate and participate in intensive case management for patients provided primarily at our 850 King St. site and in the community. This management will include assessment, treatment intervention and discharge planning. Offering person-centred care, guided by recovery principles, the Registered Nurse promotes healing and instills hope while demonstrating the values of the Mental Health and Addictions Program. This is a fast paced role that will utilize extensive knowledge of mental health illness and treatment modalities. There is on call responsibilities on a rotating basis to provide 24/7 support for client emergencies. This position contributes to ensuring there is a safe environment for patients and staff.

Qualifications:
  • Current registration with College of Nurses of Ontario (CNO)
  • Two years experience in Mental Health & Addictions Nursing required
  • Practices in accordance with Canadian Standards of Psychiatric and Mental Health Nursing
  • Certification (CNA), Psychiatric and Mental Health Nursing preferred, or willingness to obtain certification.
  • Post diploma preparation, relevant to Psychiatric & Mental Health nursing preferred.
  • Basic Rescuer, CPR, Advanced de-escalation, Code White training preferred (training will be provided)
  • Working knowledge of psychiatric and mental health disorders and related psychiatric/ psychosocial interventions and treatment models
  • An understanding of the philosophy and principles of Recovery
  • Excellent knowledge of the Mental Health Act and the Health Care Consent Act and other relevant legislation
  • Comprehensive knowledge and experience in administration and/or monitoring of psychotropic medications
  • Demonstrated ability to provide in depth mental health assessments, risk assessments, mental status examinations, and development of crisis, and discharge plans
  • Demonstrated ability to work within an interdisciplinary team setting
  • Excellent interviewing skills
  • Excellent verbal/written communication skills
  • Physical ability to perform the functions of the position which include home visits
  • Willingness to provide on call coverage on a rotating bases
  • Valid driver's license and access to vehicle required
  • Good attendance record
