As a member of Peter Munk Cardiac Centre team, the Cardiac Triage Coordinator facilitates referrals for patients requiring cardiac procedures; liase with a wide array of administrative and clinical team members throughout the organization; reviews cases with appropriate specialty physicians; prioritizes and triages patients for procedures according to urgency; ensures that patients receive timely access to cardiac procedures; ensures all necessary tests are scheduled and completed so patients can have procedures in timely manner; inputs and maintains quality patient data in Provincial COR Health Ontario database and is responsible for month end reconciliation and reporting; networks with partner hospitals, referring physicians and CCN; provides information to facilitate physician to physician communication; provides patients and families with education and support while waiting for procedure; applies best practice guidelines to ensure that patients receive care according to provincial standards; participates in Provincial Program development and implementation within University Health Network; performs cross-functional responsibilities and other duties consistent with the job classification, as assigned. REQUIRED - Current registration with the College of Nurses of Ontario - Bachelor of Science in Nursing; Masters preferred - Current BCLS Certification - Minimum Three (3) to Five (5) years Cardiology/C.V. Surgery Nursing experience - Cath Lab and/or CCU experience - Strong organizational and time management skills - Excellent interpersonal and customer service skills - Excellent verbal and written communication skills - Demonstrated ability to cultivate and build relationships - Strong track record of high quality work and engagement is essential - Able to adapt to a constantly changing environment that requires ongoing revaluation of priorities - Demonstrated ability to use good judgment in assessing difficult situations - Able to work collaboratively within a group setting - Proven track record of taking initiative - Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code.
