Reporting to the Director of Care of the Long Term Care residence and in a collaborative relationship with the senior management team to plan, organize, direct, and coordinate resident care activities within approved policies and procedures, including orientation and continuing staff education, staff health and safety and infection control programs.

The responsibilities of the role include but are not limited to:

Resident Relations:
  • Ensures residents' care is evaluated continuously. Ensures that care plans are developed, updated regularly and followed consistently.
  • Works closely with the Unit Charge Nurse to ensure that the appropriate documentation is in place as per policy.
  • Audits resident care plans on a quarterly basis and ensures documentation accurately reflects the resident's care requirements.
  • Responds to resident or family concerns and ensures appropriate action is taken within decision-making authority and/or brings to the attention of the Director of Care.
  • Ensures inventory control and promotes continuity of infection control practices with the residence, health agencies and community.
  • Ensures professional communication through membership in nursing associations and allied health organizations.
  • Responsible for leading quality improvement in nursing and personal care to monitor and continuously improve the care of services to the residents.
  • Actively leads and participates in quality improvement activities, identifies and analyzes trends in resident care outcomes.

Leadership:
  • Assumes full responsibility for the operation of the department in the absence of the Director of Care/Assistant Director of Care.
  • Assumes the role and responsibilities of the Infection Control Nurse for the Long Term Care residence (see LTC-Ca-WQ-205-01-03).
  • Responsible for the residence's Infection Control Program including: leading outbreak investigation and management, organizing resident's TB/Mantoux Protocol. Coordinates annual staff and resident vaccination program, champions new trends related to infection control and quality improvement in the field of Long Term Care, tracks cold chain and orders vaccine.
  • Liaises with Regional Infection Control staff (Public Health/Health Department).
  • Act as an educational resource in relation to infection control.
  • As directed by the Director of Care, responsible for quality management as follows:
  • Supports quality improvement in nursing and personal care to monitor and
  • continuously improve the care of services to the residents.
  • Participates in strategic planning to set goals and improve the quality plan.
  • Assists and supports nursing services and all members of the multidisciplinary team as
  • needed in the planning, implementing and ongoing evaluation of the resident centered care.
  • Actively participates in quality improvement activities in the Residence Areas.
  • Conducts assigned audits and tracks specific Performance Indicators.
  • Provides leadership for evidence based practice.
  • Identifies and analyzes trends in resident care outcomes and practice to clinical standards.
  • Participates in the development of plans of intervention to address areas of growth.
  • Keeps abreast of new trends related to Infection Control and Quality Improvement inthe field of Long Term Care.

Budget & Financial
  • Assists the Director of Care to develop budgets and other reports with respect to the operation of the Nursing Department according to department policies and procedures.
  • Assists the Director of Care to ensure efficient use of the departmental supply budget and for maintaining costs within the budget allocation for the nursing department including staffing budgets and deployment of nursing staff as it relates to outbreak management. Coordinates the purchase of infection control supplies and provides input into other clinical services arranged within the residence.

Marketing & Occupancy:
  • Actively promotes, supports and assists in public relations and volunteer recruitment programs. Acts as a role model and promotes collaboration and coordination of services/programs with community agencies and other health care providers.
  • Fosters a collaborative relationship and effective, proactive communication with Ministry of Health representatives, the community, physicians, coroner, Public Health, pharmacist, health care facilities, LHIN and other appropriate agencies or organizations, as related to Resident Care Services.
  • Represents Long Term Care Residence at seminars, workshops and other appropriate meetings.

Regulatory Compliance:
  • Ensures that the Nursing Department is operated efficiently and in compliance with the Long Term Care Act and Regulations and other pertinent legislated standards and requirements.
  • Ensures that the regulations under the Occupational Health & Safety Act of Ontario are implemented.
  • Conducts rounds and unplanned inspections of the nursing areas related to Health and Safety/Infection Control practices to ensure any unsafe practices/conditions are corrected.
  • Ensures that optimal safety standards are maintained in the nursing department.
  • Ensures representation of staff on committees which affect the service.
  • Delegates responsibilities to the appropriate qualified staff according to the applicable legislation and standards of the College of Nurses of Ontario.
  • Responds to and prepares reports on issued findings by MOH Compliance Advisors and/or complaints related to nursing care service delivery.

Operational & Administrative Systems:
  • Prepares reports, briefs and evaluations on Quality Indicators, Resident Care and Infection Control as required by the Director of Care.
  • Maintains current Resident Care, Nursing Documentation, Pharmacy and Infection Control Manuals for use in the Department and ensures its effective and efficient review and implementation.
  • Participates in ensuring the highest standard of care and quality of life is provided to residents through effective supervision of the performance of nursing staff and the maintenance of the appropriate management systems (include record keeping and reporting).
  • Provides support for the implementation of new projects related to nursing and personal care programs and services using a process of planned change.
  • Leads Infection Control and Quality/Education activities.

Other:
  • Communicates effectively and appropriately with nursing staff and the staff of other departments.
  • Counsels staff on work related issues.
  • Performs other related duties as required.

The ideal candidate will possess:
  • Must have a minimum of 3 years relevant experience in management.
  • Experience with utilizing Adult Learning Principles.
  • Current certificate of competence with the College of Nurses of Ontario as a Registered Nurse; and education in nursing, nursing administration and gerontology.
  • Must have a minimum of 3 years experience in Long Term Care (diploma or certificate in organizational management is preferred).
  • Infection Control Practitioner Certification (or willingness to complete) is considered an asset.
  • A sound knowledge and background in geriatric, rehabilitation or long term nursing care including activation, nursing and human resources administration.
  • Knowledge of The Long Term Care Act, and Regulations, Homes for Special Care Act, Long Term Care Program Directives, the appropriate sections of the Health Discipline Act, Occupational Health & Safety Act, Pharmacy Act, Food & Drugs Act (Canada), Narcotic Control Act (Canada) and other pertinent Federal, Provincial and Municipal Statutes, which influence the operation of Long Term Care Residence and more specifically the nursing department.

Special Requirements
  • Must be mature and possess excellent communication, leadership and interpersonal skills.
  • Must be in good physical and mental health.
  • Must be knowledgeable in nursing process, developments and trends in the geriatric care field and have the skill to plan, coordinate and supervise a comprehensive resident care program.
  • Must possess good managerial and organizational skills.
  • Work is primarily performed in an office environment. There is considerable walking and standing associated with monitoring facility activities. There are times of substantial bending and carrying of light items. No special physical demands are required to perform the work
