Summary:

The Registered Nurse (RN) working in the Diagnostic Assessment Programs (DAP) supports patients throughout the diagnostic journey. In addition, care is provided to eligible patients receiving indwelling pleural catheters. As a member of an interdisciplinary team, the RN participates collaboratively in the review of incoming referrals, assessment, symptom management, emotional support and patient education both on the phone and in person. As well, in collaboration with the team, the RN advocates for the expedition of patient appointments to ensure timely access to care within Cancer Care Ontario targets. The RN in the DAP program is a key contact and navigator for patients throughout their diagnostic journey, facilitating referrals and linkages when needed within the Cancer Centre, GRH and the patient's home community. The RN in the DAP program participates in the development, implementation and ongoing evaluation of the activities within the Regional Cancer Program and participates and/or supports both education and research activities. The RN will provide coverage for all programs; Breast, Colorectal 1&2, Malignant Pleural Effusion and the Thoracic Diagnostic Assessment Programs. These programs are both virtual and support in person ambulatory clinics. In these programs, the RN contributes to ensuring there is a safe environment for patients, staff and visitors.

Qualifications:
  • Certificate of Registration with the College of Nurses of Ontario
  • 3 or more years experience in Oncology nursing is required
  • deSouza Foundations of Oncology Nursing Practice or equivalent oncology certification is required, or presently enrolled in
  • deSouza Nurse Navigation Certification Program is recommended
  • Canadian Nurses Association certification as a Certified Oncology Nurse (CONC) or commitment to complete CONC in a timely manner
  • Proven ability to work independently and within the multidisciplinary team
  • Working with a variety of computer systems and software applications; proficiency with MS Office, including Excel
  • Participation in education and clinical training programs
  • Experience in telephone nursing support
  • Excellent communication skills
  • Critical thinking and problem solving skills
  • Knowledge of the current Waterloo Wellington regional cancer care system and the Cancer Care Ontario Diagnostic Assessment Program an asset
  • Good attendance record
  • Demonstrated knowledge and understanding of: oncologic disease processes, patient assessment, therapeutic communication techniques, needs of the oncology patient population and patient focused care
  • Demonstrated understanding of principles of adult learning and continuous quality improvement
  • Demonstrated knowledge of case coordination within the Cancer Centre, GRH and community agencies
  • Demonstrates empathy with patients and family members
