The Registered Nurse – Chronic Disease Management and Healthy Living is a specialist in
chronic disease management and individual capacity building, and responsible for the
planning, coordination and delivery of services to specific populations living with chronic
diseases. Working in collaboration with an interdisciplinary team, the Registered Nurse –
CDM will share expertise and knowledge of chronic disease management and optimizing
health with clients and their families, as well as other health care professionals.
The purpose of the Patient Navigation Nurse role is to ensure each patient is receiving the
best care possible to meet their goals. The Patient Navigation Nurse possesses knowledge of
the health care system as a whole, social services, and community resources to provide
optimal support to patients and their families. Working in collaboration with an
interdisciplinary team, the Patient Navigation Nurse will coordinate the implementation of
an individualized care plan and will act as a resource for colleagues, patients and their
families.
