Position Summary:

The Transitional Care Unit Manager under the direction of the Program Manager of Transitional Care Programs, will build and strengthen relationships between referral sources and Integrated Care Solutions. A strong focus will be placed on relationships with hospital and facility contacts to educate their leaders about Bayshore and ICS services and the benefits of transitional care. This will be achieved through the use of clinical knowledge to educate patients about home health service eligibility and expectations. The Transitional Care Manager will manage referral intake and coordinate the initial transition to the Alternate Level of Care (ALC) program. This role will be responsible for overseeing the ALC operations in the Transitional Care Unit.
