Salary: $41.34 - $51.67 per hour

As a member of the Nephrology Team, the responsibilities of the HEMODIALYSIS VASCULAR ACCESS COORDINATOR, include but is not limited to: utilizing and applying current specialized knowledge and expertise on recent advances in vascular access practice to facilitate the effective management of in/out-patient vascular access surveillance and care; liaison between vascular surgery, interventional radiology and nephrology; involving the coordination, implementation, and evaluation of the treatment plan by participating as a consultant, educator; collaborating in the establishment and development of policies/procedures which comply with best practice guidelines; and participating in academic and research projects; performing teaching/education responsibility; participating actively in Quality Improvement initiatives; performing administrative responsibilities; performing cross-functional and/or other duties consistent with the job classification, as assigned or requested.
QUALIFICATIONS:
- Completion of a recognized Master's degree nursing program
- Certificate of competency (licensure) from the College of Nurses of Ontario
- Completion of Basic Cardiac Life Support (B.C.L.S.) program, Level II Rescuer certification
- Up to three (3) years of relevant experience including experience working in an advanced Clinical practice role
- Three (3) to Five (5) years' experience in Hemodialysis or Nephrology Nursing
- Effective organizational, interpersonal and communication skills
- Excellent decision-making and problem solving skills
- Excellent time management skills
- Excellent teaching skills with the ability to teach one-on-one and/or groups
- Ability to function collaboratively within a multi-disciplinary team and/or independently
- Ability to work in a self-directed manner
- Must be willing to travel across all dialysis sites within UHN to assess and educate about vascular access and vascular access surveillance.
- Demonstrated awareness of and adherence to professional and ethical standards
- Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand
- Computer literacy
- Research experience preferred
- Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code.
