Reporting to the Clinical Manager, Child & Adolescent Mental Health Outpatient Services, you will work primarily in our Specialized Eating Disorder Service and outpatient interdisciplinary services. The successful candidate will be responsible for collaborating with the patient, family, multidisciplinary team and other service providers to coordinate, facilitate and participate in assessment, treatment intervention, medication monitoring and management, as well as discharge planning adolescents under the age of 18 years. The nurse will also contribute to ensuring there is a safe environment for patients, staff and visitors.

Responsibilities include but are not limited to:
  • Provide ongoing health care monitoring for adolescents participating in the outpatient Eating Disorders clinic
  • Participate in comprehensive intake assessments to further ensure correct level of service access
  • Participate in all aspects of the care provided within the outpatient eating disorder clinic, where possible and as appropriate within scope of practice. This may include the following: group co-facilitation, meal support, psychoeducation, supportive individual and family counselling, program planning, best-practice research and program evaluation
  • Current knowledge and experience in administration/monitoring of medication for the adolescent population • Provides treatment recommendations, referral and discharge planning for adolescents in response to the unique and complex needs of this population
  • Able to adapt model of care interventions to provide a recovery oriented plan to provide stabilization, weight restoration, eating and symptom interruptions.
  • Works effectively with members of the multidisciplinary team in establishing and monitoring processes, improving continuity of care for patients, and enhancing collaborative relationships with other care providers
  • Participates in service committees and quality improvement initiatives as required
  • Engages in professional development and continuing education
  • Assists with program development and evaluation as required Participates in team building, service development and improvement activities striving to contribute to a healthy workplace environment
  • Other duties as assigned

QUALIFICATIONS:
  • Current registration in good standing with the College of Nurses of Ontario
  • BScN preferred
  • Three years relevant experience working within a Psychiatric Acute Care Setting required
  • Minimum two (2) years relevant nursing experience as a Registered Nurse in child and adult mental health an asset.
  • Knowledge of psychopharmacological interventions and psychiatric disorders within an adolescent population required
  • Experience with adolescent assessment and treatment modalities
  • Excellent interpersonal communication and documentation skills
  • Demonstrated commitment to family-centred care
  • Evidence of strong professional boundaries
  • Skilled at providing virtual (video and phone) care
  • Psychiatric and Mental Health Nursing certificate preferred
  • Required minimum of at least year experience working with adolescents presenting with complex mental health problems
  • Strong knowledge of the adolescent community resources preferred
  • Practices in accordance with Canadian Standards of Psychiatric and Mental Health Nursing
  • Excellent knowledge of the Mental Health Act, Health Care Consent Act and F&CS.
  • Functions effectively & collaboratively with a multi-disciplinary team
  • De-escalation training and Code White experience
  • Demonstrated ability to work with adolescents presenting a wide range of emotional and behavioural problems required
  • Demonstrated skills in crisis assessment, crisis planning and risk management required
  • Good attendance record
