Summary:

Offering person-centered care, guided by recovery principles, the Registered Nurse promotes healing and instills hope while demonstrating the values of the Mental Health and Addictions Program. Adult Transition Team or ATT is a best practice model of providing community specialized mental health care to people with severe and persistent mental illness. This program is located at Freeport and supports people living in all types of housing options across Waterloo Region. The team will be supporting patients recently discharged from Specialized Mental Inpatients and require a team to support them until they can be transitioned to a permanent team such as FACTT or ACTT. This clinician will assess the client's individual status and needs, support the treatment plan and, in collaboration with all ATT members. The Registered Nurse will provide the injections and monitoring of medications and required lab work associated with the medication. The ATT team utilizes a client centered approach striving to support the client's recovery plan. Contributes to ensuring there is a safe environment for patients, staff and visitors. Registered Nurse will work in collaboration with the community partners and housing providers.

Qualifications:
  • Current registration with College of Nurses of Ontario (CNO)
  • 3 years experience in Mental Health & Addictions Nursing - working with people who have a severe and persistent mental illness required
  • Certification (CNA), Psychiatric and Mental Health Nursing preferred, or willingness to obtain certification.
  • Experience in Community Mental Health preferred
  • Post diploma preparation, relevant to Psychiatric and Mental Health nursing preferred.
  • Advanced de-escalation, Code White training preferred (training will be provided)
  • Basic Rescuer, CPR
  • Working knowledge of psychiatric and mental health disorders and related psychiatric/ psychosocial interventions and treatment models
  • An understanding of the philosophy and principles of Recovery
  • Working knowledge of current best practices related to treatment of Severely Mentally Ill persons.
  • Comprehensive knowledge and experience in administration and/or monitoring of psychotropic medications
  • Valid Ontario Driver's License and access to motor vehicle required
  • Excellent interviewing skills (individual/group)
  • Demonstrated group facilitation skills
  • Strong skills in biopsychosocial assessments, care planning, crisis plans, psychiatric nursing interventions and complete outcome evaluation for individual care.
  • Demonstrated ability to provide in depth mental health assessments, risk assessments, mental status examinations, and development of crisis, and discharge plans
  • Case Management experience preferred, with ability to develop strong community agencies relationships
  • Working knowledge of the Mental Health Act and Community Treatment Order Legislation, Privacy Act, Health Care Consent Act, and other relevant legislation.
  • Knowledge of Community Resources preferred
  • Strong consultation skills with Physicians, Community therapists, Support workers and families.
  • Ability to work independently with consultation from multidisciplinary team members
  • Demonstrates excellent interpersonal skills, including ability to engage in conflict resolution
  • Excellent critical thinking, organizational and problem solving skills
  • Good attendance record
