We currently have a Regular Full-time opportunity for a Registered Nurse in the Geriatric Ambulatory CPSE Mental Health at our Bayview campus.
Hours of Work:
Weekdays: Days; 8 hrs; As per schedule
Summary of Duties
Acts as a nursing resource for elderly, psycho geriatric patients suffering with mental health illness. The incumbent will function within an interdisciplinary team assisting with conducting mental status evaluations, collecting collateral information, providing follow up counselling, facilitating referrals, sourcing outpatient and community resources, and provides health teaching to clients, families and in some cases staff. Practices in accordance with Sunnybrook's Patient Focused Care philosophy.
This position requires comprehensive knowledge of community agencies, the health care system and the GTA geography.
A valid driver's licence and reliable car are a necessity as extensive travel within the GTA is foundational to the position.

Qualifications/Skills
- Current College of Nurses of Ontario registration required
- Current C.L.S. Certificate required
- Baccalaureate degree in Nursing required
- Non-violent Crisis Intervention certification required
- Previous psychiatric nursing experience required
- Psychiatric/Mental Health Nursing Certification preferred
- Expertise with the Geriatric population
- Prefer GEM designation with Psychiatric specialization
- Knowledge of system functioning and navigating resource/ access to community agencies , retirement homes, primary practices, Long Term Care Facilities, and CCAC
- Comfortable teaching health care to elderly populations and families as well as staff in agencies
- Excellent interpersonal and communication skills and ability to establish therapeutic alliance with patients with various mental health diagnoses
- Ability to conduct a nursing mental status evaluation
- Experience assessing patients with severe mental illness in crisis
- Comprehensive knowledge of psychotropic drugs
- Comprehensive knowledge of senior's community resources and community mental health resources for seniors.
- Demonstrated understanding of Patient-Focused Care and the ability to utilize the principles in daily practice
- Ability to problem solve, prioritize and manage time effectively
- Ability to work effectively both autonomously and within an inter-professional team environment
- A valid driver's licence and reliable car necessary for home visits to elderly in our catchment area
- Competent with varied injection techniques as required for administering long acting psychotropic medications
