Position: CLINICAL NURSE SPECIALIST (CNS) - PSYCHIATRIC- MENTAL HEALTH
Department: PETER MUNK CARDIAC PROGRAM
Reports To: CLINICAL DIRECTOR

Salary: $44.95-56.19 per hour
Site: TORONTO GENERAL HOSPITAL
Hours: 37.50 HOURS/WEEK - DAYS - MONDAY to FRIDAY
Status: TEMPORARY FULL-TIME - 12 month contract

University Health Network (UHN) is looking for an experienced professional to fill the key role of Center for Mental Health Clinical Nurse Specialist (CNS)_within the Peter Munk Cardiac Center outpatient areas at the Toronto General Hospital (TGH).

The CNS will play a key role on the multidisciplinary cardiac team in the outpatient areas. The CNS will take part in the launch of a pilot project with the goal of reviewing patient -reported outcome measures including PHQ-9 results & acting on them based on their risk assessment. Furthermore, the CNS will undertake mental health assessments of cardiac patients seen in the cardiology clinics at TGH. They will also support the inter-professional care team in developing appropriate clinical interventions as well as providing support and advice to the patient and significant others. This role will provide expertise in developing and monitoring care plans for medically and behaviorally complex individuals. In addition, the CNS will build capacity with staff around addressing psychiatric and behavioral issues in the outpatient setting and consult with other mental health and addiction services as required.

The CNS will have an important educational role relating to patient and clinical staff and will be responsible for building capacity with staff and family members.

The primary focus of this role will be on mental health care of cardiac patients.

The principal responsibilities include:
  • Utilizing and demonstrating a comprehensive theoretical knowledge base and advanced level of clinical competence in caring for cardiac patients involving, collecting and interpreting data about the mental health of the patient, determining a mental health diagnosis and treating complex responses of individuals and families to actual or potential problems.
  • Playing a leadership role and performing quality improvement and administrative responsibilities.
  • Guiding, initiating and providing leadership in policy and procedure development to advance care and patient experience.
  • Engaging in evidence-informed practice by critically appraising and applying research and best practice guidelines and performing research activities.
  • Delivering, coordinating and participating in education and professional development activities/programs.
  • Acting as a resource and serving as a consultant to individuals and groups within the professional community and other hospital/agencies.
  • Working in close collaboration with the Centre for Mental Health (CMH) C-L (consultation-liaison) psychiatry team
  • Collaborating with PMCC and Centre for Mental Health team (physicians and advanced practice nurses) to ensure continuity of mental health care for cardiac patients


QUALIFICATIONS:
  • At minimum, a completion of a Master's degree in nursing
  • Additional preparation in the clinical specialty required
  • CNA Specialty Certificate in Psychiatric Mental Health Nursing (CPMHN(c)) preferred
  • Completion of a Basic Cardiac Life Support (B.C.L.S.) program
  • CPI (Crisis Prevention Institute) Certified or willing to obtain
  • Current Registration with the College of Nurses of Ontario as a Registered Nurse (General Class)
  • Minimum 5 years recent related clinical experience in psychiatric and mental health nursing
  • Prior experience as a Clinical Nurse Specialist in a specified clinical specialty program is preferred
  • Advanced knowledge of the interplay of psychiatric, addiction, and medical or surgical conditions
  • Advanced knowledge of psychotherapeutic modalities and behavioural interventions provided at the medical/psychiatric interface
  • Knowledge of professional trends, issues, scope and standards of practice, and other mental health related relevant legislation
  • Demonstrated theoretical and empirical knowledge bases and advanced level of clinical competence in working with patients with mental health and/or substance use disorders
  • Advanced knowledge of psychopharmacology
  • Highly developed communication skills both written and oral
  • Previous experience in designing or facilitating teaching/educational initiatives
  • Excellent interpersonal and team collaboration skills
  • Demonstrated ability to act as a resource and consultant to clinicians and interdisciplinary teams
  • Demonstrated self-directed, critical thinking and problem solving skills
  • Demonstrated organizational, presentation and leadership skills
  • Demonstrated commitment to patient safety and quality improvement
  • Demonstrated commitment to continuous learning and ongoing professional development
  • Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand
