We currently have a Regular Part-Time (0.2 FTE) opportunity for a Nurse Clinician -Geriatric Emergency Management (GEM) in the Geriatric Emergency Management department at our Bayview campus.

Hours of Work:

Weekends: Days;8 hrs; As per schedule

Summary of Duties:

The Nurse Clinician will be a member of the interprofessional team providing assessment, care, and treatment in the Emergency Department. In collaboration with other team members, the Nurse Clinician will:
  • Provide consultation to health care professionals and elderly patients regarding geriatric health issues
  • Assess, plan, implement and evaluate care of elderly persons with complex geriatric health problems
  • Collaborate with health care professionals to develop and implement a
  • comprehensive plan of care
  • Assess the coping strengths of the elderly person and his/her family/support and plan intervention and/or intervene to assist the elderly person to achieve optimal independent functioning

Qualifications/Skills:
  • Bachelor's degree in Nursing required. Master's degree in Nursing or Gerontology is an asset.
  • Current College of Nurses of Ontario Certificate of Competence; current B.C.L.S.
  • Experience working in community health care
  • Working knowledge of community resources, LHIN, and RGP
  • Experience in specialized geriatric programs is an asset.
  • Additional training and/or experience in Gerontology is required
  • Knowledge of Comprehensive Geriatric Assessment (CGA)
  • Excellent oral and written communication skills
  • Demonstrated ability to function on an interdisciplinary team
  • Knowledge and skills in functional assessment and the assessment of physical and mental illness in the elderly
  • Excellent organizational skills, problem solving and time management skills
  • Ability to adapt to change, innovate, and be flexible in an ever-changing environment
