Scale: F0:03
Salary: $80,613 - $100,757 per annum (Commensurate with experience and consistent with UHN Compensation Policy)
Hours: 37.5 Hours/Week; Monday to Friday, Day shifts (may require some flexibility)
Status: Permanent Full Time

Position:

As an integral member of the inter-professional team, the Patient Care Coordinator (PCC) acts as a clinical resource for the inter-professional teams. The PCC has clinical and leadership expertise in the care for Spinal Cord Injury patients. The principal responsibilities include playing a leadership role in providing direction, ongoing development for nursing staff and other inter-professional team members in the clinical areas, performing financial/administrative responsibilities to ensure efficient and effective unit operation; contributing to staff development by promoting and ensuring a supportive/collaborative learning environment based on the principles of adult learning and practice standards for staff and students; performing human resources responsibilities in collaboration with the clinical program services manager; liaising with members of the multi-disciplinary team to ensure high standards of quality and optimal management of patient care outcomes (including data collection/reporting) and ongoing continuous improvement practices to strengthen clinical processes. The PCC will be reporting to the CCC Program Services Managers.

Qualifications:
  • Registration with the appropriate regulatory College
  • Baccalaureate degree in one of the required Health Professions; Masters degree (or equivalent) is preferred
  • Basic Cardiac life Support (B.C.L.S.) program required
  • Three (3) years of relevant clinical experience, including experience with complex medical patient management is required
  • Demonstrates membership within a professional association and/or professional interest groups related to practice preferred
  • Demonstrated strong leadership and motivational skills, with ability to helping staff understand the relationship to their work and organizational goals
  • Demonstrates ability to work in a team environment and collaborates with others in assisting with the delivery of care to patients
  • Demonstrated leadership abilities
  • Excellent skills in recognizing, analyzing and resolving problems
  • Excellent time management skills
  • Excellent decision making and problem solving skills
  • Clinical and patient teaching skills
  • Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards
