We currently have a Regular Part-Time (0.4 FTE) opportunity for an Advanced Practice Nurse in the Palliative Care Services Department at our Bayview Campus.

Hours of Work:

Weekdays: Days; 8 hrs; As per schedule

Summary of Duties

The Advanced Practice Nurse (APN) is a registered nurse who has acquired the expert knowledge base, complex decision-making skills and clinical competencies for expanded practice. This advanced practice role involves analyzing, synthesizing and applying nursing knowledge, theory, and research evidence to foster system-wide changes and advance nursing care and the profession as a whole (CNA, 2008). The APN contributes to the quality of patient care through the roles of advanced practitioner, educator, researcher, administrator, and leader within an interprofessional environment. The emphasis on each of these domains will vary based on the organizations priorities.

The APN will be responsible for developing and promoting evidence-based clinical practice with a person and family centred focus; teaching and supporting the professionalism and practice of nurses as well as other disciplines across the organization;

Most specifically the APN will be expected to work directly with the In-patient Palliative Care Consult team clinically two days a week crossing all acute care units and providing consultation to patients and their families on various pain, symptom management and psychosocial issues. As well as following up with patients who have been discharged.

Requiring the ability to communicating and working collaboratively with referring services and the interprofessionals teams on acute care units.

The successful candidate will:
  • Engage in both direct and indirect clinical care (focus on palliative care issues)
  • Demonstrate and facilitate system leadership
  • Mentor, support, and work to advance nursing practice with a palliative care focus
  • Engage in evaluation and research, and Quality Improvement Initiatives
  • Develop, promote, and role model evidence-based and best clinical practices
  • Lead and champion the integration of person & family-centered care initiatives
  • Collaborate effectively with interprofessional health care team members
  • Be open to meeting the ever changing needs of patient care and organizational Priorities

Qualifications/Skills
  • Required current Ontario College of Nurses Certificate of competence and BCLS certificate.
  • Master's degree studies in Nursing or health-related discipline (must be completed).
  • 5 years previous Oncology or Palliative Nursing experience required
  • Canadian Nurses Association certification as Certified Oncology Nurse CON(c) or Oncology Nursing Society (OCN) certification considered a definite asset.
  • Canadian Nurses Association certification in Hospice & Palliative care also considered an asset.
  • Demonstrated effective leadership and team work abilities.
  • Proven well-developed organizational, time management skills; flexible and adaptable to change and adjusting priorities.

  • Excellent interpersonal and communication skills (oral, written and comprehension)
  • Demonstrated commitment to patient centre care approaches and principals
