We currently have a Regular Full-time opportunity for a Nurse Practitioner in the Schulich Heart Centre Primary Responsibility: Aortic Valve Program at Sunnybrook.

Hours of Work: Weekday: Days; 8hrs - some flexibility required

Summary of Duties:

The Nurse Practitioner (NP) is an advanced practice role designed to contribute to the quality of patient care through practice, research and education. The NP promotes wellness and health promotion and provides exemplary quality care through the application of advanced clinical and theoretical knowledge and skills. The NP practices in a manner that reflects the Mission, Vision and Values of Sunnybrook and in compliance with the professional standards of nursing practice as defined by the hospital and College of Nurses of Ontario.

The role will be engaged in coordinating the clinical management of patients with aortic stenosis who 1) require assessment of eligibility for a structural heart procedure such as Transcatheter Aortic Valve Interventions (TAVI) and Surgical Aortic Valve Replacement (SAVR) 2) require post procedural care and follow-up upon completion of a procedure. In addition, the role will provide clinical care to structural heart patients in both inpatient and outpatient settings.

The Nurse Practitioner will collaborate with patients, family members and other health care providers in the provision of safe high-quality patient centred care. As an advanced practice nurse, the NP will also collaborate with the Patient Care Manager, Clinical Educator and Advanced Practice Nurse to enhance nursing practice through the development, implementation and evaluation of best practices.

Qualifications/Skills
  • Current College of Nurses of Ontario Certificate of Competence;
  • Master's degree with Nurse Practitioner Certification;
  • Demonstrated clinical expertise in cardiac surgery or cardiology preferred;
  • Demonstrated clinical expertise in geriatric and chronic conditions;
  • Current B.C.L.S.;
  • Membership in R.N.A.O.;
  • Previous experience as a Nurse Practitioner required
  • Program planning and evaluation skills;
  • Demonstrated professional leadership and ability to be innovative and creative;
  • Excellent interpersonal and communication skills.
  • Expertise in conflict management
