We currently have an exciting opportunity for a Full time Foot Care Nurse to join our Ambulatory Medical Program.
Responsibilities:
- Conducts detailed and advanced foot care assessments
- Develops and implements individualized foot care treatment plans
- Determines and implements appropriate advanced foot care nursing interventions
- Manages common and advanced foot conditions, with a particular focus on diabetic foot conditions
- Manages and conducts all aspects of foot care clinic operations according to established RNAO best practice guidelines for diabetic foot care, CNO standards for Infection, Prevention and Control and Canadian Diabetic Association foot care guidelines
- Liaises with the Diabetic Education Centre, Humber River Hospital Dialysis populations, and Inpatients to ensure at risk patients have access to timely foot care resources
- Liaises with the Inpatient Skin & Wound specialist to ensure continuity of care following discharge
- Provides health teaching to the patient/caregiver related to appropriate foot care practices and self-management at home
- Establishes linkages with community resources and connects patients with the appropriate supports
- Co-ordinates and ensures all supplies/related equipment is available, properly maintained and fully operational
- Maintains all related patient care documentation in accordance with set guidelines, including collection and submission of evaluation data
- Develops foot care educational materials for patients and staff
Requirements:
- Current Ontario College of Nurses Registration in good standing
- Current BCLS certification or demonstrated competency.
- Completion of Basic, Advanced and Diabetic Foot Care certification through a recognized certification program.
- Minimum 2 years recent experience in advanced foot care.
- Demonstrates knowledge of chronic disease management.
- Ability to be adaptable and work effectively in a fast-paced environment with changing priorities.
- Must possess excellent organizational skills
- Demonstrates excellent health teaching skills that encompass adult learning principles and strategies.
- Evidence of sound judgment and initiative.
- Demonstrates effective problem solving skills and an ability to work well independently with minimal supervision.
- Conducts oneself with professionalism and respect at all times.
- Highly effective interpersonal and communication skills are required.
- Sensitivity to and awareness of cultural, racial, economic and socially diverse communities
- A positive attendance record
- Proficiency in a second language used in the community served would be an asset
