COME WORK WITH US!

Our Diabetes Care Team is looking for a Registered Practical Nurse (RPN) to provide foot care support across all of our locations. Working as a member of a multidisciplinary team, the RPN delivers comprehensive foot care including analysis of health behaviours related to lifestyle and culture, review of client’s medical history, foot care assessments, and in collaboration with the client, development and implementation of client-centred care plans.

REQUIRED QUALIFICATIONS

  • Postsecondary diploma (RPN) from an accredited college
  • Current registration with the College of Nurses of Ontario
  • Specialized training in foot care and Footcare Certificate
  • Knowledge of diabetes and a minimum of 3 years’ experience supporting and educating clients who have diabetes
  • Ability to conduct initial assessments and develop individualized foot care plans
  • Experience with monofilament test assessments, foot care recommendations, diabetic foot care teachings, and foot care treatments
  • Knowledge and experience with proper reprocessing procedures
  • Willingness to manage and conduct all aspects of footcare visits both onsite and in clients’ homes
  • Ability to work with diverse communities with respect, sensitivity, creativity, innovation, cultural understanding, and non-judgement

PREFERRED SKILLS

  • Awareness and understanding of the social determinants of health and health promotion
  • Excellent written and oral communication and interpersonal skills
  • Experience with Electronic Medical Records (Telus Practice Solutions Suite currently in use)
  • French and other language skills are considered an asset in supporting the communities we serve
