Responsibilities of the Nurse Practitioner at Mill Creek Care Centre

In this role as a Nurse Practitioner, you will work collaboratively with the team including physicians, RN's, RPN’s, PSW’s, and other direct care staff to provide care for residents. 

Essential Duties are the technical skills and knowledge that an employee requires in order to perform their role. Below is detailed information on required Essential Duties for your position.

  • In collaboration with the Medical Director and Attending Physicians, provides comprehensive primary care to residents

  • Collaborates and consults with physicians, the Director of Care, nursing staff, interdisciplinary team members and external resources regarding residents’ plan of care.

  • Assists, supports, guides, and provides consultation to the Director of Care, nursing staff and interdisciplinary team members regarding challenging clinical situations.

  • Makes referrals to specialized consultants, services, and other health providers

  • Performs a person-centered health assessment of residents on admission, annually and as needed (including medication reconciliation and quarterly medication reviews)

  • Conducts person-centered comprehensive health histories and participates in regular care conferences

  • Orders and/or performs appropriate screening and diagnostic investigations, interpreting results and assuming responsibility for follow-up

  • Diagnoses acute and chronic health conditions

  • Manages the care of residents by providing pharmacological, complementary and/or counseling interventions, and performs procedures within the NP scope of practice

  • Advocates for and provides palliative and end of life care

  • Documents clinical data, assessment findings, diagnoses, plans of care, therapeutic interventions, resident responses, and clinical rationale in a timely and accurate manner

  • Provides person-focused health education

  • Collaborates with the resident, family, and the interdisciplinary team to develop a person-centered plan of care

  • Provides outreach and transitional services to LTCH residents who return to the community

  • Liaises with discharge planning services for hospitalized residents to ensure a smooth transition back to the LTCH

  • Participates in providing after hours and on-call coverage in accordance with the on-call policy of the LTCH

  • Identifies and implements research-based innovations for improving resident care and collaborates with members of the interdisciplinary team and/or community to identify research opportunities and to conduct and/or support research

  • Other duties as assigned.

Qualifications

  • Current registration with the CNO as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice (required),

  • Experience working as a Nurse Practitioner,

  • Experience in long-term care, with providing empathetic care for the elderly, 

  • Experience providing leadership and supervision in any healthcare environment.

  • Must have a clear vulnerable sector screen, no older than 6 months, TB test results less than 6 months old, must be able to provide three supervisory references.

  • Mill Creek Care Centre recognizes the importance of immunization to protect our residents, staff, and others from COVID-19. As such, subject to any accommodation required by applicable human rights legislation, it will be a condition of employment that all new hires are up to date with their eligible COVID-19 boosters approved by Health Canada, 14 days before start date.

Mill Creek Care Centre (MCCC) is a charitable, non-profit, Christian home for residents needing LTC living. MCCC Long Term Care Home is located within Tollendale Village. Our home is strongly grounded on Christian traditions and principles, while welcoming residents of all faiths, backgrounds and beliefs. Mill Creek Care Centre is home to 160 residents. Our home has 5 Resident Home Areas (RHA) which provides living accommodations for 32 residents on each home area.

 

It is situated within Tollendale Village at 286 Hurst Drive in Barrie on the south side of Kempenfelt Bay. The Village is in close proximity to the downtown area and is accessible by the municipal bus services.
