Job Summary:

The Manager Home and Community Care - Regulated provides professional and practice leadership and is responsible for ensuring the provision of safe, effective quality client care. Coordinates and manages the delivery of in home care services, staff training, programs, and supervision of all front line staff and team operations. The position is accountable for executing site operational plans, and ensuring adherence to all policies and practices.

This role will support Sarnia-Lambton and Chatham-Kent.

Responsibilities:

  • Supervision and performance management of front line service providers
  • Directs and monitors delivery of client care services and day to day functions of front line staff in accordance with current regulatory guidelines and VON policies, procedures and standards and assists with the delivery of in home care services, as required
  • Facilitates staff development and growth using a team approach and working in concert with the practice leaders, educators, preceptors and clinical experts to facilitate excellent outcomes for quality care delivery, client service and provider safety, and service provider competence. This includes and is not limited to use of the following resources: competency assessments, annual performance development plans, individual learning plans, referral to practice educators and on-going mentorship
  • Receives and documents all employee, family and client complaints/issues related to front line service delivery and involves other management personnel in preparation and execution of resolution plan, as required
  • Assists RN in completing risk assessments, investigation and resolution of issues, incidents or injuries
  • Ensures occupational health and safety policies, programs and activities are performed by all front line staff in compliance with VON Canada Health and Safety policies and guidelines and the Occupational Health and Safety Act
  • Participates with annual budget preparation, performance management and measurement reports
  • Regularly monitors performance related to funder expectations and contributes to the development and implementation of quality improvement activities to achieve performance targets
  • Regularly liaises/communications proactively and constructively with various members of the following teams:
  • District home care management team
  • Local and district management team
  • Greater VON clinical management team; and
  • Functional teams
  • May participate on any local, regional or national team or committees as required, desired or requested. Examples include, Quality Care and Service, Joint Occupational Health and Safety, Accreditation, Recruitment and Retention
  • Effectively use evidence based decision-making metrics
  • Build external relationships as assigned by Site or District Executive Director, and may include but are not limited to: client organizations such as the LHIN, CCAC, District Health Authorities, Ministries of Health and community health, social and service agencies
  • Participates in the on-call rotation
  • Other duties as assigned

Qualifications:

  • Bachelor's Degree in Nursing (BScN)
  • Current CNO registration in good standing
  • Minimum of 5 years' experience leading staff in a community nursing
  • environment including: knowledge of information and communication technology and practices, budget management, decision making, coaching, negotiation, and program or project management
  • A current and original copy of a satisfactory Criminal Records Check is required upon the start of your employment
  • A Vulnerable Sector Search and/or Child Abuse Registry Check may be required
  • Ability to speak French is an asset in French Designated areas.
  • Must have personal vehicle and possess both a current driver's license and proof of vehicle insurance
