Position: Registered Nurse

Department: Heart Function Clinic

Status: Permanent Part-time (must provide availability of up to a 0.6 FTE)

Hours: Monday to Friday - 8 hour day shifts, 0800-1600 (hours are subject to change based on operational needs).

This is what you'll be doing:

The Heart Function Clinic Registered Nurse is responsible for performing assessments, developing a plan of care and implementing and evaluating the care of patients with Heart Failure in coordination with the Cardiologist and Pharmacist.
  • Performing clinical assessments of the new and returning patients to the clinic
  • Utilize the physical assessment and critical thinking in assessing patient symptoms or lifestyle changes in conjunction with heart failure guidelines and protocol to determine appropriate patient care plan
  • Coordinate the plan for patient care between the Cardiologist and Pharmacist assessments
  • Provide education to the patients regarding lifestyle and dietary adjustments associated with the heart failure conditions
  • Work collaboratively with the patient and the patient family to promote the appropriate condition management
  • Document the assessment information in the clinic specific information management systems according to the given guidelines for patient information management

  • Current registration in good standing with the College of Nurses of Ontario
  • Current BCLS-HCP certification required at time of hire
  • Demonstrated clinical expertise in care of patients with Cardiovascular disease is required
  • Demonstrated knowledge of Heart Failure is required
  • Demonstrated knowledge of ECG interpretation is an asset
  • Demonstrated initiative and organizational skills
  • Demonstrated teaching skills
  • Experienced in verbal and communication skills
  • Satisfactory attendance and performance record
  • Physically able to perform the job

Salary: $34.24 to $49.02 per hour
