Status: Temporary Full-Time for up to 1 year

Hours: Monday to Friday, days, 7.5 hour shifts. Subject to change based on operational requirements

Site: Multi-site (Oakville, Milton, Georgetown)

Overview:

As a member of our inter-professional team, the Nurse Practitioner will utilize clinical expertise and in-depth knowledge of children, adolescents and their families as well as adults to plan and evaluate patient care. The successful candidate will provide leadership and promote excellence in nursing by providing expertise in clinical practice, consultation, education and research in the area of addiction. The Nurse Practitioner will integrate research evidence into practice to ensure best practice standards are maintained and incorporated into clinical initiatives.

This is what you will be doing:

The principal responsibilities of this role are to identify, screen, assess, consult and provide clinical intervention to individuals with Substance Use Disorders and their families. This will include providing specialized assessment, coordination and treatment for individuals living with Substance Use Disorders. The Addiction and Concurrent Disorders (or, potentially, simplified as Addiction) Clinician provides counselling, education, and support services to individuals, families and the community, helps clients understand and address substance use and related problems, including comorbid physical and mental health issues, problem gambling, family dynamics, and lifestyle changes. The Addiction Clinician is expected to work collaboratively with an inter-professional team within the Mental Health Program and to establish and maintain partnerships with related community organizations. This role will be part of the addiction services program within Mental Health.

Halton Healthcare Services is seeking individuals who are leaders in their chosen profession. The successful candidate must be a natural team player with effective communication skills. They must be patient focused, caring, respectful and fiscally responsible. The successful candidate will have:
  • Master's Degree Program in Nursing/Master of Science in Nursing or recognized equivalent is preferred
  • Current registration in the Extended Class with the College of Nurses of Ontario is required, preferably with the Primary Health Care specialty certificate
  • Nurse Practitioner's Certificate from a recognized program
  • Current membership in professional association such as CNA, RNAO or NPAO preferred
  • Completion of the Opioid Dependence Treatment Certificate or a willingness to obtain in the next 1 year.
  • Minimum three years' experience working in the field of substance use disorders/addiction.
  • Thorough knowledge of substance use and associated disorders, cause and effects, evidence-based models of treatment intervention and their application to diverse populations.
  • Expertise in conducting bio-psychosocial assessments and strong formulation skills to design person-cent red, strength-based change strategies that reflect evidence-based practices and clinical experience.
  • Experience in motivational interviewing and trauma-informed care.
  • Provide opioid addiction treatment with a harm reduction approach to care.
  • Provide integrated model of care that will include linking clients to primary care and/or fostering existing relationships with primary care to ensure client needs are being met.
  • Knowledge of individual, family, group, and crisis counselling methods.
  • Demonstrated skill in working with families of those who have substance use disorders.
  • Demonstrated skill and knowledge about concurrent mental health and substance use disorders.
  • Established comfort working across the continuum of a harm-reduction framework.
  • Substantiated experience working with inter-professional teams, under pressure and within a fast-paced hospital setting.
  • Demonstrated understanding of diverse cultures and the principles of cultural safety.
  • Confirmed experience developing and/or facilitating groups for those with Addiction & Concurrent Disorders.
  • Experience in matching treatment services and interventions to clients' and their families' unique circumstances whenever possible.
  • Experience in motivational interviewing and trauma-informed care.
  • Work in partnerships with primary care, community addiction and withdrawal management providers.
  • Ability to work independently.
  • Proficient with MS Office applications and related electronic health care platforms
  • Satisfactory employment and attendance record required
  • Experience in the provision of psychoeducation and psychotherapy is preferred


Salary: $51.530 to $63.010
