Job Description
Place of employment: Long Term Care

Responsibilities:
  • Participates actively in the Continuous Quality Improvement Program
  • Ensure resident centric care is planned and provided in accordance with the Standards of the College of Nurses, the Long-Term Care Act and Best Practice Guidelines.
  • Contributes to and attends Admission, quarterly and Annual Resident care reviews
  • Develop an individualized resident care plan and incorporate a collaborative interdisciplinary approach in this process, using RAI-MDS and Point Click Care
  • Excellent assessment and clinical skills in providing direct resident care
  • Assists with respect to delegation of tasks to staff, emergency response, conflict resolution and management of the nursing unit.
  • Actively participates in Risk Management and associated committees (falls, wounds, restraints, etc.)
  • Maintains good relationships with Medical/Support/Home, staff, residents, families and the public
  • Utilizes leadership skills to identify problems and problem-solving on the unit
  • Monitors that care delivered is consistent with the resident Bill of Rights.
  • Profession Values and Ethics- Protects the rights of the residents, families in matters such as: confidentiality, privacy, beliefs, and values.
  • Ensures that ethical guidelines are met in accordance with the standards set out by the College of Nurses
  • Reporting of ethical issues to the Assistant Director of Care /Director of Nursing
  • Is aware of employee's responsibilities and follows all Health and Safety policies and procedures.
  • Works safely to reduce the risk of injury to self, co-workers, and residents
