For over 60 years Sunbeam Community & Developmental Services (former, Sunbeam Centre) has been committed to serving children, youth, adults, and seniors living with complex developmental, intellectual, behavioural, and/or physical disabilities. Based in Waterloo Region, with services offered across Waterloo, Wellington, Dufferin, Halton and Peel, Sunbeam is a transfer payment agency through the Ministry of Children, Community and Social Services (MCCSS), and has charitable and non-profit status.  Sunbeam provides a broad range of programs, supports and services to the individuals served and their families.

Community Living Services (CLS) is one of three operating divisions of Sunbeam.  CLS operates 19 residential care homes (group homes) and overnight respite sites, as well as a day programming, passport funded activities and a variety of therapeutic support services, resources, and amenities for residential and community clients.

Key Duties:

 

 

  • Conduct comprehensive assessments for clients and assist with the creation and on-going updating of individualized PCSP’s which incorporate current medical needs, physical and mental health status, history, preventative health care strategies and ongoing health monitoring.  
  • Assess, plan, implement, document, evaluate, and revise plan of health care for individual clients according to Physician's orders, and Person-centered Support Plans on an ongoing basis.
  • Conduct formal and informal healthcare teaching to regulated and non-regulated healthcare providers, to research a wide range of health related topics, to develop staff training packages, present workshops/seminars, and evaluate the learners’ mastery of the information (including the teaching, delegation and monitoring of controlled acts).
  • To work collaboratively with The Multi-Disciplinary Team to identify, predict and prevent concerns, through diagnostic problem analysis, data collection, ongoing progress assessment, evaluation and the development of a relationship based on mutual respect of competency.
  • To assist and support Staff & families to respond to the changing needs of clients with developmental disabilities who are aging and experiencing declining health.
  • To provide direct healthcare teaching, to clients with developmental disabilities, about understanding their illnesses & bodies, nutrition, sexuality, medications, etc.  
  • Assist in the development & implementation of agency policies and procedures related to health/medical practices incorporating and CNO Professional Standards and Best Practice Guidelines.
  • Advocating for directives when client care may be compromised; advocating for changes in directives to reflect current evidence-based practice.
  • Review medication errors, providing direction, support, and re-training as necessary.
  • Co-chairing medication reviews and pharmacy audits for group homes.
  • Manage, schedule, and administer client immunizations as per "Canadian Recommended Immunization Schedule" and assist with organizing and implementing annual Influenza vaccine to clients and staff.
  • Participates in continuing education, to maintain and expand current knowledge and skills; increasing skill sets in response to changing health needs of clients; developing annual individualized learning projects to ensure contemporary competencies.
  • Reviews client Incident Reports and provide direction for direction where injury has occurred or could be prevented.
  • Participate in orientation and on-going support and training of new and existing regulated and unregulated staff.
  • Perform a liaison function between the agency and community physicians, pharmacists, and other Health Disciplines, coordinating referrals, consultations, and follow-up appointments.  Coordinate client Annual physicals and In-House physician assessments.
  • Perform other related duties as required.

 

 

Qualification Criteria:

 

 

 

  • The successful incumbent will be highly motivated, energetic possess a positive attitude and be able to mentor and motivate others. He/she will have excellent leadership skills and encourage and deliver a high standard of care and support to all persons served.
  • Demonstrate effective communication skills, responding in a professional, respectful manner with clients, family, all staff of Sunbeam Centre and the members of the Multi-Disciplinary Team, promoting collaboration, respect & cooperation throughout the agency.
  • Current certificate of registration as a Registered Nurse in Ontario Current CPR/First Aid certificate B.Sc. N. or Nursing Diploma with relevant related experience Five years nursing experience using a consultative model in a community setting Broad base of clinical experience including: Extensive knowledge of range of developmental disabilities and associated risk factors and disorders experience with the needs/issues of children and adults with developmentally disabilities with concurrent complex, psychiatric &physical challenges, across the full health-illness spectrum
  • Health care teaching/training experience: training needs assessment, research, course development and delivery, learner evaluation

Demonstrated strong interpersonal skills, effective verbal/written, communication and consulting skills Strong computer skills and familiarity with electronic documentation Excellent attendance record, dependable, dedicated, compassionate and flexible. Ability to work independently and as an integral part of a multi-disciplinary team Valid driver's license and reliable transportation        

 

Hours of Work:
