Key Qualifications:
- Certified Diabetes Educator
- Membership in the Diabetes Educator Section of Diabetes Canada preferred
- Minimum 5 years’ experience in counselling clients with diabetes on an individual and group basis
- Good working knowledge of diabetes and comorbidities through the lifecycle, adult teaching principles and patient engagement strategies
- Good working knowledge of diabetes medications, including insulin
- Highly effective decision making, problem solving and organizational skills combined with excellent interpersonal and communication skills, both oral and written
- Proficiency in computers, Microsoft Office and experience with electronic medical records and good keyboarding skills are essential
- Reliable, flexible, creative professional, able to work effectively and collaboratively in a multidisciplinary team approach required
- Experience using medical directives an asset
- Additional language an asset
- Positive attendance and work record essential
- Member in good standing with College of Nurses of Ontario
Position Summary:
- Responsible for development of nursing care plans and counselling for patients with diabetes and/or co-morbidities such as renal, cardiovascular disease and mental illness
- Responsible and accountable for decisions and actions in the provision and management of nursing care for patients with diabetes in a collaborative inter-professional team environment
- Broad understanding of resources available in the community to help support caseload of patients through critical thinking and problem solving to ensure best practices are maintained
- Assist and support clients to gain the knowledge, skills, attitude and self-awareness necessary to effectively manage his/her diabetes
- Monthly rotations in the inpatient units at the Mississauga Hospital to support bedside teaching for diabetes and safe discharge.
- Function as a part of a multidisciplinary team, and act as a resource for other health care providers and the community
- Using a variety of teaching methods, incorporating the principles of adult education and a variety of client engagement or empowerment modalities to facilitate behaviour change (ie motivational interviewing, stages of change )
- Facilitates group education programs for patients as needed
- Develops appropriate education materials to meet the needs of the clients
- Assists in program planning and evaluation
