Position: Nurse Practitioner – NSM Trans Health Program (Maternity Leave Contract – No benefits - Possible end date of January 2023), Reports To: Director of Clinical Services

Position Summary:

As a key member of the Couchiching Family Health Team (CFHT), and an integral part of an interdisciplinary health care team, the major role of the Nurse Practitioner will help to improve access to primary care for transgender individuals in the NSM Region. As a member of an interdisciplinary regional team, the NP will provide comprehensive health care to trans individuals in the community, ensuring continuity of care and access to specific healthcare needs, such as hormone therapy and surgical referrals. Through ongoing education and consultation, the Nurse Practitioner will also work to build the capacity of health care providers in the region around trans health needs.

1)Trans and Gender Diverse Primary Care

2)Transition Related Surgeries: Planning, Referral and Care

3)Weekly Mentorship Calls (Q and A)

CAMH and University of Toronto

Roles and Responsibilities:

• Responsible for providing healthcare services for trans-identified clients of all ages

• Assess holistically and provide services to trans clients in all developmental stages

• Complete health assessments, including health history and physical examination

• Formulate and communicate medical diagnoses

• Determine the need for, and order from, an approved list of screening and diagnostic laboratory tests and interpret the results

• Monitor the ongoing therapy of clients with chronic stable illness by providing effective pharmacological, complementary or counselling interventions

• Prescribe drugs according to scope of practice

• Provide health education and health promotion information to individuals and groups

• Consult with Primary Care Practitioners in accordance with the standards for consultation with physicians, and/or refer the client to another health care professional

• Ensure all notes and client charts are updated in a timely manner

• Prepare quarterly reports and monitor daily statistical data to ensure statistics are reported to the Ministry of Health in a timely fashion

• Participate in CFHT and regional committees as needed

• Provide education on trans health-related issues to Primary Care Practitioners and service providers

• Maintain and upgrade knowledge and skills

• Maintain current knowledge of policies and procedures

• Maintain confidentiality in accordance with the Municipal Freedom of Information and Protection of Privacy Act

• Work in compliance with Health and Safety laws adhering to safe work practices and procedures established by the CFHT

• Other duties as required

Skills/Knowledge Requirements:

• Demonstrated ability to work effectively as a member of an Interprofessional team, with excellent interpersonal and communication skills

• Exceptional documentation skills and attention to detail

• A positive and professional attitude

• Excellent time management skills with an ability to multi-task and effectively set priorities

• Knowledge of trans population issues and resources and the Family Health Team model as asset

• Adaptable to change

• Knowledge in current, evidence-based methods and practice of health care delivery.

• Excellent oral/written/interpersonal skills

Education/Specific Job Requirements:

• Current registration with the College of Nurses of Ontario as an RN-EC (extended class) – Primary Health Care

• Current registration with Nurse Practitioner Association of Ontario or Registered Nurses Association of Ontario preferred and evidence of Professional Liability Insurance coverage

• Certification in CPR (BLS-C)

• Baccalaureate Degree in Nursing

• Primary Health Care Nurse Practitioner Certificate

• Familiar with the World Professional Association for Transgender Health (WPATH) Standards of Care

• Familiar with the Guidelines and Protocols for Hormone Therapy and Primary Health Care for Trans Clients (Sherbourne Health Centre)

• Current Ontario Driver’s License and access to reliable vehicle for travel to various communities

• Provide evidence of a clean driver abstract and proof of insurance

• Able to work flexible hours including evenings if required

• Police Record/Vulnerable Record Check

• It is a condition of employment that you receive the necessary vaccinations to protect against infection of COVID-19, or any similar infectious diseases, which may be recommended by governmental and public health authorities from time to time

Experience:

• Preference will be given to candidates with 2-3 years clinical experience as a Nurse Practitioner, preferably with the trans population

• Experience in program development an asset

• Experience with an Electronic Medical Record (EMR) and office applications.
