We currently have a Regular Full-time opportunity for a Social Worker in the 7 East Nursing Unit at our Holland campus.

Hours of Work:

Weekdays: Days; 8 hrs; As per schedule

Summary of Duties

To provide a full range of social work services to patients and their families at the Holland Orthopaedic and Arthritic Centre. This includes psycho-social assessments, counselling to patients and families, discharge planning, crisis intervention, linkage to community resources, team consultation.

Qualifications/Skills
  • Masters Degree in Social Work
  • Member of the Ontario College of Social Workers and Social Service Workers
  • Minimum two (2) years of experience working in a Hospital setting
  • Previous experience working with musculoskeletal surgery, geriatric / elder care and end of life
  • Cognitive support experience required
  • Excellent assessment, interviewing and problem solving skills
  • Excellent written and oral communication, mediation, time management and organizational skills
  • Strong team building skills
  • Knowledge of the health care system and community resources
  • Superior ability to manage multiple tasks and set priorities in a fast-paced environment
  • Commitment to person centred care
  • Commitment to student education Exceptional communication skills required
  • Familiarity with issues of capacity and consent
  • In-depth understanding of the Sunnybrook Discharge Policy and its application
  • Strong negotiation and conflict resolution skills
  • Ability to be a team player as well as self-directed
  • Ability to manage multiple tasks and set priorities in a fast-paced environment
  • Commitment to student education
