POSITION SUMMARY: Under the general supervision of the Clinical Manager and the direction of the registered nurse on duty assumes the team leader role of the Resident Home Area including resident care, staff, and the environment within the RPN scope of practice; provides optimum individualized resident care in accordance with the Standards of Nursing Practice, College of Nurses of Ontario, and long-term care legislation.

MAJOR RESPONSIBILITIES:
  • As a valued member of the team, the Registered Practical Nurse demonstrates a commitment to resident safety by providing quality care in accordance with the Pioneer Ridge vision, mission, and caring philosophy.
  • Plans, implements, coordinates, and evaluates the nursing care of the residents to meet their physical, psychological, emotional, cultural, spiritual, and social needs.
  • Completes all required documentation including assessment, care plans, medical documentation, MDS and RAI.
  • Encourages resident participation and family support in activities of daily living and participates in the provision of health promotion.
  • Creates a comfortable and safe environment that provides for opportunities for privacy, dignity, autonomy, independence and individuality and maintenance of personal lifestyle, values, and resident rights. Demonstrates clinical skills by recognizing the importance of maintaining resident independence through promotion and encouragement of their involvement in Activities of Daily Living.
  • Guides and directs Personal Support Workers and other staff on the Resident Home Area to ensure delivery of optimum care to residents.
  • Fosters an atmosphere that is conducive to the provision of optimal care, and the effective functioning of the assigned Resident Home Area through collaboration with other multi-disciplinary team members.
  • Facilitates relationships between and amongst residents, relatives, staff, and the community.
  • Exercises sound nursing judgment by reporting any change in residents' status in a timely manner to the Registered Nurse as necessary, and takes appropriate action.
  • Partakes in the Pioneer Ridge sustainable RAI MDS 2.0 by completing the assigned assessments (Coding, Raps and Care Planning) within the required timelines.
  • Ensures the strategies and interventions directed by the care plan are implemented in a safe and effective manner.
  • Provides guidance and direction to new staff, students, and volunteers; participates in their orientation to Pioneer Ridge.
  • Responsible for medication administration and management, narcotic control, timely and accurate transcription of physician orders, preparation and involvement in resident care conference, and preparation and care of resident at end of life and deceased individuals.
  • Ensures the effective and efficient monitoring and maintenance and use of equipment and supplies in compliance with policies and in adherence to approved budgets.
  • Performs such other related duties as may be assigned.


  • QUALIFICATIONS:

    Education/Experience:

    • A current unrestricted Certificate of registration as a registered practical nurse with the Ontario College of Nurses and proof of current Basic Cardiac Life Support.
    • Knowledge of long-term care legislation; specifically, the Long Term Care Home Act as well as the Residents Bill of Rights


    Skills/Abilities:
    • Ability to communicate effectively and concisely, both orally and in writing
    • Proven ability to establish and maintain good working relations with superiors, co-workers, residents, families, and the general public
    • Strong leadership skills, including the ability to motivate others and to share skills and knowledge with others
    • Ability to identify, understand, and meet/exceed the requirements of internal and external customers
    • Competent within the meaning of the Occupational Health and Safety Act

    Assets:
    • Experience in Gerontological Nursing would be an asset


    PHYSICAL DEMANDS: This position requires constant walking/standing; frequent squatting/crouching; unilateral lifting 1-25 lbs. frequently, bilateral lifting 50+ lbs. frequently, 15 - 75 lbs. occasionally; push/pull 5 - 100 lbs. frequently.

    Occasional = up to 33% of working day; Frequent = 34-66% of working day; Constant = 67-100% of working day.

    CONDITIONS OF EMPLOYMENT:
    • Will be required to maintain current certifications as listed under qualifications
    • Must provide proof of full COVID-19 vaccination at point of conditional offer
    • Prior to commencing employment, will be required to review and attest/affirm to all Pioneer Ridge Staff Immunization Policies
    • Undertake professional development in order to maintain competencies in the long-term care area and complete all mandatory education as per legislation and policy
    • Must be physically able to perform the essential duties of the position
    • Must undergo a successful Police Vulnerable Sector Check (PVSC)
    • Must have successfully undergone a "two step" tuberculosis skin test within the 12 months prior to employment
