Registered Nurse – Safer Opiate Supply (SOS)

Full-Time, Contract Position to March 31, 2023

Salary Range:  $65,362 - $76,897; Includes HOOPP (Healthcare of Ontario Pension Plan) 

 

South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. In response to the ongoing opioid crisis and as a complement to the Downtown East Collaborative - Safer Opiate Supply Program (DEC-SOS), SRCHC is looking to build a large interdisciplinary mobile team of providers who offer prescriptions for safer opioid supply as well as therapeutic and supportive treatment for people who use substances in the further east part of Toronto. Team members will work closely and collaboratively with colleagues, partners as well as service users with a focus on addressing the needs of equity-deserving communities in a variety of neighborhoods where overdose risks are high and resources limited. This role requires the flexibility to work in a variety of settings on a daily basis.

 

As an integral member of the interdisciplinary SOS team and working closely with SOS Nurse Practitioners, the Registered Nurse works to full scope in the provision of safer supply and primary care support for individuals receiving SOS services. Conducting screening and intake assessments for potential clients will be required with this position.

 

This position will be required to work some flexible hours, including some evenings and weekends and will also be involved in shelter outreach work and working with clients who are COVID-19 positive. Working onsite during the COVID-19 pandemic is required.

 

Responsibilities:

  • Conducts comprehensive health assessments
  • Assesses and manages episodic and chronic conditions commonly seen in primary health care
  • Provides supportive education and counselling which may include preventative counselling, brief supportive counselling or short-term crisis management when indicated
  • Assesses the need for prescriptions, laboratory and diagnostic tests within the RN scope of practice
  • Participates in clinical care at off-site locations/partner agencies as required
  • Makes internal referrals to other SRCHC staff and programs as appropriate
  • Makes external referrals to specialists, hospital or community resources as appropriate
  • Participates in case conferences with other providers to ensure coordination of services to the client; works as a member of an interdisciplinary team
  • Participates as a team member in all aspects of program planning, implementation and evaluation
  • Ensures that standards of client care are maintained according to accepted College of Nurses standards, the Regulated Health Professions Act, and the policies of SRCHC
  • Follows established documentation protocols and maintains clear, accurate and timely written documentation in accordance with procedures and policies of the College of Nurses of Ontario, Ministry of Health and Long-Term Care and SRCHC
  • Prepares/maintains statistical data, correspondence reports and other documentation, as required
  • Performs necessary back-up clinical functions for other clinical staff
  • Incorporates health promotion and harm reduction strategies and recognizes the determinants of health, as well as the social and structural inequities that create the conditions of the social determinants of health
  • Participates in the development of, and gives support to, policies/strategies which are in the interest of public health and which support the recognition of the determinants of health
  • Responds to medical emergencies in accordance with scope of practice
  • Assesses, responds to and manages overdose events; provides overdose prevention education
  • Distributes harm reduction supplies and provides harm reduction education
  • Provides basic primary care to address injection drug use and other substance use issues
  • Collaborates with own team and other teams across the organization to identify and provide health education to target groups
  • Other duties as required

Qualifications:

  • BSc in Nursing from a recognized university
  • Current registration as a Registered Nurse with the College of Nurses of Ontario
  • 3 – 5 years in a community setting or combination of community, hospital or public health settings
  • Experience in primary care or street/outreach nursing an asset
  • Significant frontline nursing harm reduction experience; values aligned with a harm reduction approach to service delivery
  • Experience with overdose prevention and response and training in/experience with Naloxone administration
  • Physical ability to provide support in an overdose situation
  • Thorough current nursing knowledge and proficiency in conducting a comprehensive health history, physical assessment and construction of a nursing management plan
  • Demonstrated thorough knowledge and understanding of infection control and preventative practices
  • Knowledge and experience of primary care issues and challenges for high risk communities
  • Knowledge of community services and resources; strong advocacy skills
  • Demonstrated display of sensitivity and empathy to others
  • Demonstrated ability to de-escalate conflict
  • Confidence in asserting and maintaining strong boundaries
  • Demonstrated ability to work effectively in a multi-disciplinary environment
  • Demonstrated commitment to and knowledge of community- based health care
  • Non-judgmental and non-confrontational attitude
  • Demonstrated respect for diversity, equity and accessibility with the ability to work collaboratively and positively with others
  • Excellent interpersonal skills
  • Excellent organizational skills
  • Excellent verbal and written communication skills
  • Proficiency in the use of computer technology and various software applications
  • Must be willing to work within a harm reduction, Safe Supply program in conjunction with SRCHC’s Consumption Treatment Services and COVID-19 positive sites
  • Commitment and ability to work with equity deserving communities
  • Demonstrated experience working across all genders with racialized populations, indigenous peoples, women who engage in sex work, individuals who are/have been incarcerated, people with mental health challenges, and people who have experience trauma, considered an asset
  • Police clearance documentation will be required for employment
  • Proof of triple COVID-19 vaccination status will be required for employment
