Job Description
ROLE SUMMARY:

This position has to be comfortable to travel to different locations if needed.

Possible locations - Etobicoke, Woodbridge, Aurora, Downtown Toronto
  • Hourly Rate: $29.50 to $31.00
  • Full-Time

The Registered Practical Nurses are integral to helping Safehaven achieve its goals of providing the highest in quality of care to children with complex medical needs and dependent on life-sustaining medical technology.

Reporting to the location Client Care Coordinator, a key responsibility of this role is to provide supervision, care and programming for clients who are medically complex and are reliant on life sustaining medical technology and equipment in a residential setting. This provision of care includes direct care, medication administration, residence upkeep, recreation and programming, cooking and cleaning for the clients.

As part of the interdisciplinary team, you will practice within your full scope to plan, organize and provide care to patients in accordance with the nursing philosophy and according to the standards of nursing practice of the College of Nurses of Ontario and within the Regulated Health Professionals Act legislation. Specifically, you will apply knowledge of nursing process, as it relates to scope of practice, to achieve identified patient goals when overall care needs are less complex, outcomes are predictable and risk of negative outcomes is low.

A required duty of this role is the provision of care, and technological assistance, to a client population with a tracheostomy/ventilator, requiring specific technologically complex care practices and procedures.

RESPONSIBILITIES:
  • Provide for the daily living needs of medically complex and technologically dependent clients by maintaining a safe environment with appropriate attention given to individual health and care needs, nutrition, hygiene, and recreational/program planning
  • Act as the primary caregiver for at least one (1) client, taking on full care, responsibility, and oversight of all aspects of the client's life at the Safehaven locations (only for residential clients as opposed to respite clients), including needs specific training and education, documentation, doctor/medical visits, General Service Plan (GSP) documentation and maintenance, parental involvement/communication, preventative maintenance, and direct involvement in all aspects of the client's life, etc.;
  • May be required to receive training and instruction in the following secondary roles: Occupational Health and Safety, Scheduling, Respite, Ordering, Property, Fire drills, Documentation, or Recreation;
  • Using a framework for nursing practice in accordance with the Ontario College of Nurses, assesses client's needs, collects and compiles data, identifies problems, develops, communicates, implements, evaluates care plans and reports significant changes in medical conditions;
  • Assess both the client and the family/guardian needs, collects and compiles data, identifies problems, develops, communicates, implements and evaluates care plans;
  • Consult and collaborate with interdisciplinary team members to ensure a coordinated plan of care to meet client and family/guardian needs;
  • Respond respectfully and effectively to people of all cultures, classes, races, ethnic backgrounds, sexual orientation and faiths and religions in a manner that recognizes, affirms and values the worth of individuals, families and communities;
  • Maintain client records by collecting and documenting information including health care information regarding client and their family/guardian, nursing observations, telephone communications, interventions, teaching and care evaluations to ensure that resident records are current and complete
  • Participate in admissions and discharges of clients, ensuring all information is documented;
  • Administer medications and maintains logs on medicine administration;
  • Responsible for cleaning and disinfecting the physical environment, as aligned with the Safehaven policies and procedures, recording any concerns and documenting that cleaning has been completed as needed/specified
  • Follow all health directives as required or assigned;
  • Under the direction of the Client Care Coordinator, designs and implements individualized program plans, and other interventions, in consultation with program consultants, other employees, family/guardians within the scope of practice outlined by the Ontario College of Nurses;
  • Accompany clients to therapeutic and recreational appointments as requested by the Client Care Coordinator;
  • Liaise with community services, schools, and the family/guardians of clients as required;
  • Attend employee meetings and appointments as required;
  • Participate in the orientation and training of new employees, placement students, and volunteers as directed by the Client Care Coordinator;
  • Assume responsibility for day-to-day coordination of employees, and overseeing of the operation of the location, in the absence of the Client Care Coordinator (i.e. vacation, leave of absence, and illness), acting as the In-Charge employee as per schedule;
  • Cooperate with all employees in establishing a supportive working environment in the residential setting;
  • Follow all policies and procedures, established by the organization as they pertain to the operation of the residence, and the health, safety and welfare of clients in care;
  • Maintain the safely of staff and clients by practicing medical asepsis, using the clients' safety devices and using mechanical lifts.

QUALIFICATIONS:
  • Registered Practical Nurse with current certificate of competence from the College of Nurses of Ontario (CNO);
  • Two (2) - Three (3) years related experience working with medically complex clients who are reliant on life sustaining technology and equipment;
  • Successful completion of the following Sick Kids' Learning Institute or University of McMaster Connected Care 3 Part Module:
  • Paediatric Tracheostomy
  • Central Venous Access Devices: Care and Maintenance in the Home
  • Home Ventilation & Pulmonary Clearance
  • Other training/education as required by the medical complexity of the client
  • Ability to efficiently arrange and perform a wide range of tasks;
  • Ability to work under the pressure of a large and busy household;
  • Ability to perform delegated tasks alone with minimal supervision;
  • Ability to assume additional training in the therapeutic needs of the clients, nutrition, pharmacology, CPR, first aid training, seizure identification and response, and Non-Violent Crisis Intervention (NVCI) training;
  • Possesses good team work, problem solving and written/verbal communication skills;
  • Ability to respond to a flexible work schedule involving shifts and/or emergency staffing needs in the residence and/or emergency needs of the clients in care.
Company DescriptionFor over 25 years, The Safehaven Project for Community Living has been providing exceptional care to children with developmental disabilities and complex care needs. Funded by the Ministries of Community and Social Services and Children and Youth Services, we opened our first residential homes in North York, Aurora and Etobicoke in 1991. In 2001, Safehaven added a central location at 1173 Bloor Street West, which is entirely dedicated to providing respite services to families throughout the GTA. In 2004, we opened an additional six bed residential home in Woodbridge.

In 2020, Safehaven served over 200 disabled individuals and their families, including permanent residents living at our Aurora, Clayland, Glenaden and Timber Lane homes and those who utilized respite services at our Bloor, Aurora, Glenaden and Timber Lane locations.
