Job Description
Role Summary:

The Respite Care Coordinator/Registered Nurse is responsible for coordinating the respite program and ensuring that clients are placed with providers that have the appropriate qualifications including experience with disability care or providing support to those with special needs. This role is responsible for respite staff, maintaining a database, and billing respite allocations.

Reporting to the Chief Executive Officer and working as a part of the Shared Services team for all Respite needs for Safehaven, to implement and sustain quality of care and service delivery.

  • Provide appropriate, high quality support to enable people with disabilities to enjoy a positive lifestyle as a respected member of their local community;
  • Conducting outreach to potential referrals sources to obtain clients for Respite Program and achieving and maintaining appropriate utilization goals;
  • Developing and maintaining client medical and case management reports in compliance with confidentiality and HIPAA requirements;
  • Preparing program reports as required;
  • Ensuring that client care plans appropriately identify client needs and identifies the levels of health and independence the client can achieve;
  • Ensuring that potential clients are screened and client intakes are conducted in appropriate and safe manner;
  • Provide client service needs through case management and referrals services;
  • Always acts in a respectful and effective manner with all people, in a manner that recognizes, affirms and values Safehaven;
  • Upholds a high standard of proper conduct and professionalism;
  • Follows Health and Safety policies that are set out at Safehaven, demonstrating a positive "health and safety" attitude;

  • Bachelor's degree in healthcare, management or related fields;
  • Generalized background with broad knowledge of residential care for persons with medical complexities;
  • Knowledge of behavioral health practices and procedures; process facilitation; supportive counselling and case management activities
  • Strong written and verbal skills, organizational excellence, and team-oriented;
  • Excellent computer skills, particularly Microsoft Office Suite, and Excel specifically;
  • Must have problem solving skills and the ability to perform under pressure while retaining excellent quality of service
  • Ability to work with the Safehaven leadership team, while also being able to work directly with the front-line staff to solve issues and problems as they occur;
  • Exhibit good judgment, practical day-to-day experience working with a diverse group of people, and hardworking
Company DescriptionFor over 25 years, The Safehaven Project for Community Living has been providing exceptional care to children with developmental disabilities and complex care needs. Funded by the Ministries of Community and Social Services and Children and Youth Services, we opened our first residential homes in North York, Aurora and Etobicoke in 1991. In 2001, Safehaven added a central location at 1173 Bloor Street West, which is entirely dedicated to providing respite services to families throughout the GTA. In 2004, we opened an additional six bed residential home in Woodbridge.

In 2020, Safehaven served over 200 disabled individuals and their families, including permanent residents living at our Aurora, Clayland, Glenaden and Timber Lane homes and those who utilized respite services at our Bloor, Aurora, Glenaden and Timber Lane locations.
