Basic Function:

This position is to support all aspects of the COVID-19 team related tasks and to ensure that the requirements of the Health Protection and Promotion Act and all associated legislation are met by providing quality programs and services. To contribute to health promotion and disease prevention initiatives to individuals, families and groups; to provide nursing services for individuals, families and groups according to the program-specific policies and procedures, and the program's operational requirements; to meet the professional practice standards as defined by the College of Nurses of Ontario; and to meet and adhere to the current Canadian Community Health Nursing Standards of Practice, and the Core Competencies as defined by the Public Health Agency of Canada.

Position Description:

Health Promotion - integrates health promotion into practice, using strategies including:
  • Identifies strategies for change that will make it easier for people to make healthier choices.
  • Uses a client centered approach to help the individual, family and group to identify strengths and available resources to access health and take action to address their needs.

Prevention and Health Protection - integration of prevention and protection activities, using strategies including:
  • Recognizes the differences between the levels of prevention (primary, secondary, tertiary) and selects the appropriate level of intervention.
  • Facilitates informed decision making for protective and preventive health measures.
  • Helps individuals, families and groups to identify potential risks to health. Be knowledgeable about specific emergency/disaster plans and promoting awareness of the plan(s) amongst individuals, families, groups and communities

Demonstrates ability to fulfill functional roles in response to a public health emergency as part of an intra-professional nursing team. Health Maintenance, Restoration and Palliation - integration of systematic and planned methods to improve health and support using strategies including:
  • Assesses the health status and functional competence of the individual, family or group
  • Develops mutually agreed upon plans and priorities for care with the individual, family or group
  • Maximizes the ability of an individual, family or group to take responsibility for and manage their health needs according to resources and personal skills available.
  • Supports informed decision making: acknowledges diversity, unique characteristics and abilities; and respects the individual, family or group's specific requests.
  • Adapts community health nursing techniques, approaches and procedures to health challenges and the challenges related to equity in health in a particular community situation or setting.
  • Autonomously facilitates maintenance of health and the healing process for the individual, family or group when overall care needs are less complex, outcome are predictable an d risk of negative outcomes is low including in response to significant health emergencies or other community situation that negatively impacts health.
  • Evaluates outcomes systematically and continuously in collaboration with the individual, family or group including other health practitioners.

Uses knowledge of the community to link with and refer to community resources

Professional Relationships - building and nurturing professional relationships using strategies including:
  • Assesses individual, family or groups beliefs, attitudes, feelings and values about health and health inequities and their potential effect on the relationship and intervention
  • Recognizes her or his personal beliefs, attitudes, assumptions, feelings and values about health and their potential effect on interventions/strategies.
  • Is aware of and uses culturally relevant communication strategies when building relationships. Communication may be verbal or non-verbal, written or graphic. It may involve face-to-face, telephone, group, print or electronic methods.
  • Respects, trusts and supports or facilitates the ability of the individual, family or group to identify, solve and improve their own health issues.
  • Recognizes and promotes the development of health enhancing social support networks as an important determinant of health.
  • Maintains awareness of community resources, values and characteristics
  • Negotiates an end to the relationship, in a professional manner, when appropriate (i.e., when the client demonstrates readiness and assumes self-care, when the goals for the relationship have been achieved, or based on the direction of the organization/employer).
  • Evaluates the nurse/client relationship as part of regular practice assessment.

Capacity Building -building individual and community capacity using strategies including:
  • Works collaboratively with the individual, family or group (including other health care providers) to identify needs, strengths, available resources and strategies for action.
  • Engages the individual, family, or group in a consultative process from a foundation of equity and social justice
  • Recognizes and builds on the readiness of the individual, family, or group to participate and act.
  • Uses empowering strategies such as mutual goal setting, visioning and facilitation.
  • Helps the individual, family or group, to participate in issue resolution to address their determinants of health.
  • Actively shares knowledge with other professionals and community partners and appreciates the importance of collaborative team work.

  • Applies the principles of social justice and advocates for those who are not yet able to take actions for themselves.
  • Uses a comprehensive mix of strategies to address unique needs and to build individual, family, or group capacity.
  • Evaluates the impact of change on the health outcomes of the individual, family or group.

Access and Equity - facilitation of access and equity for equal distribution of resources using strategies including:
  • Assesses and understands the capacity of the individual, family or group.
  • Identifies and facilitates universal and equitable access to available services.
  • Collaborates with colleagues and with other members of the health care team and community partners to promote effective working relationships that contribute to comprehensive client care and optimal client care outcomes.
  • Provides culturally sensitive care in diverse communities and settings
  • Support the individual, family or group's right to choose alternate health care options
  • Advocates for equitable access to health and other services and equitable resource allocation.
  • Adapts practice in response to the changing health needs of the individual, family or group.
  • Refers, coordinates or facilitates access to services in the health sector and other sectors.

Professional Responsibility and Accountability- demonstration of responsibility and accountability through strategies including:
  • Assesses and identifies risk management issues and takes preventive or corrective action individually or in partnership to protect individuals, families and groups from unsafe, unethical illegal or socially unacceptable circumstances.
  • Identifies ethical dilemmas about whether responsibility for issues lies with the individual, family or group or with the nurse or the nurse's employer.
  • Makes decisions using ethical standards and principles, taking into consideration one individual's right over the rights of another, individual or societal good, allocation of scarce resources. Seeks help with problem solving, as needed, to determine the best course of action when responding to ethical dilemmas, risks to human rights and freedoms, new situations and new knowledge.
  • Advocates for societal change to support health for all based on the concepts of health equity and social justice.
  • Identifies and acts on factors which affect practice autonomy and delivery of quality care.

Requirements

Knowledge and Experience:
  • Current Certificate of Annual Registration with the College of Nurses of Ontario as a Registered Practical Nurse.
  • Minimum two years' experience in nursing, preferred.
  • Current certificate of CPR

Skills and Abilities:
  • Proficient oral and written communication skills required.
  • Valid Ontario driver's license and access to a reliable vehicle as travel is required within Haldimand and Norfolk Counties.
  • Computer expertise in corporate standard software (Microsoft Office) and department specific software to data search and entry and to create and modify word processed documents
  • Must be aware of safe work practices as they relate to job responsibilities and work environment, and have the basic understanding of the Occupational Health and Safety Act


Benefits

Status: Temporary Full Time (date of hire to approximately June 30, 2022)
