A new mobile outreach Inter-professional Care (IPC) team has been established to bring primary care, case management, mental health, system navigation and harm reduction services to homeless and precariously housed individuals in the Moss Park Community. This innovative initiative is a partnership with Sherbourne Health and Street Health. The IPC team will utilize Sherbourne’s Health Bus and provide low threshold health care in the evening and weekends in the Moss Park Community.
about the role:
You are a collaborative individual who will liaise as required with other members of the IPC team and external health care providers to ensure the delivery of comprehensive health care to clients on the Health Bus and complete follow up at Sherbourne Health Centre and in the community as required.
day to day responsibilities:
As the NP, you will provide a full scope of primary care and understands the impact of the broad determinants of health on people’s ability to attend to their health care needs. In this role, the NP works to full scope of practice and working closely within a multidisciplinary team to provide care throughout the client’s life span through a variety of methods including: comprehensive nursing assessments, health promotion and education, outreach to marginalized individuals and groups, advocacy, and community development.
to thrive in this role, you’ll need:
• Current registration as an RN (EC) with the College of Nurses of Ontario
• Three years experience working with persons marginalized by social location including homelessness, poverty, sexual identity, gender identity, residency status
• Experience obtained in either primary care or a community setting is preferred
• Experience working with individuals who face mental health and substance use issues
• Knowledge and understanding of primary prevention and health promotion theory and practice
skills and abilities:
• Proven ability to work within a multidisciplinary team and also independently
• Strong interpersonal and communication skills
• Demonstrates an understanding of working within an anti-oppression framework
• Ability to speak languages in addition to English is an asset
conditions of employment:
• Employment is contingent upon the successful completion of a Police Records Check
• Vulnerable Sectors check will be initiated upon first day of employment.
• Commitment to a healthy and safe workplace for self and others by taking all reasonable precautions and working in compliance with SHC related policies, health and safety legislation and best practices.
• Understand and maintain compliance with the Occupational Health and Safety Act (OHSA), Personal Health Information Protection Act (PHIPA), Accessibility for Ontarians with Disabilities Act (AODA), the Ontario Human Rights Code and other relevant legislation.
• The NP works primarily on the Sherbourne Mobile Health Bus.
• Follow-up at Sherbourne Health or other sites in the community when applicable.
• This role will require work primarily in the afternoon, early evenings and weekends.
