Assistant Director of Care - Registered Nurse
Assists the Director of Care of the Long Term Care Home (and in a collaborative relationship with the management team) to plan, organize, direct, and coordinate resident care activities within legislative Acts + Regulations as set out by our governing bodies.
The primary responsibilities of this position include but are not limited to:
For the Director
Assist the Director of Care in clinical and operational responsibilities of the home.
Conduct internal inspections and reporting audits to identify areas of improvement.
Perform supervisor on call duties and responsibilities.
Designated to act for the Director of Care in their absence.
Assist in the recruitment, orientation, training and disciplining of staff when necessary, while ensuring compliance with appropriate collective agreements. Facilitate training towards team building.
Drafting letters for transfers, terminations, and letters to CNO.
Prepare reports as directed and maintains/monitor appropriate statistics.
Conducting performance reviews.
Works collaboratively with the Director of Care on special projects, reviews, and other duties as may be assigned.
Is the team lead for various committees (Customizing Medications, Continence Care, Palliative Care).
Is a member of various other committees (Falls, Behaviours, Wound Care, QI … etc).
For the Residents
Maintains an open line of communication with residents and families and address any concerns.
Support and embrace resident and family centered care.
Provide exceptional customer service to residents and their families, following the values of person-centered care.
Completes assessments for residents being transferred from home and being admitted to home.
Develops plans of care with the resident and their caregiver that suits their needs.
Monitor and reassess resident’s needs and ensures that resident's care is evaluated continuously.
Assists residents to access resources available through various community partners.
Participates in multi-disciplinary care conference.
Assists with completing investigations.
Develop and maintain continuous quality improvement initiatives.
For the Staff
Provide direction and guidance to the Registered Staff and PSW's
Support, coach and mentor others.
Support RAI MDS programs
Performs Occupational Health and Safety duties when necessary
Is an active member of Joint Health and Safety Committee.
Employee engagement with new hired staff through regular follow up for staff retention
Familiarity with collective agreements
Monitors modified work programs for nursing personnel
Ensures that all assigned staff receives proper orientation, in-service education and on-the-job training.
Registered Nurse; BScN preferred
Registration with College of Nurses of Ontario in Ontario and a member in good standing
Minimum of three 3 experience in long term care; including past experience demonstrating progressive leadership and hands-on approach to monitoring and evaluating resident care
Post graduate studies in nursing, nursing administration and geriatrics are an asset
Demonstrated superior leadership and organizational/interpersonal skills; excellent verbal and written communication skills
Computer skills: Microsoft Word; Excel, Power Point, computerized resident care and assessment programs
Strong clinical knowledge with a focus in gerontology in order to provide direction and guidance to the registered staff and PSW's
Knowledge of the Acts, Bills and Regulations as set out by governing bodies (MOHLTC, RHRA, Ontario Law etc)
Occupational Health and Safety Act is an asset
Strong knowledge of RAI MDS and Point Click Care (PCC)
