Nurse Practitioner - full time/permanent

Summary:

As a key member of the North Peel Family Health Team (NPFHT), the Nurse Practitioner (NP) will deliver nurse practitioner care in accordance with NPFHT policies and professional practice standards, and according to regulations set out by the College of Nurses of Ontario. The NP will perform nurse practitioner duties and ensure that the primary care needs of NPFHT patients under his/her care are met.

The Nurse Practitioner’s primary role is to work in partnership with NPFHT Physicians and other health care providers/ support staff to perform clinical patient care.

Accountability:

  • The Nurse Practitioner (NP) reports to, and is responsible to, the Executive Director. All clinical decision making outside the scope of the NP must be done in consultation of the NPFHT Lead Physician and/or physicians of NPFHT and/or medical directives.

Authority:

  • Does not direct the activities of staff or a function without direction of FHT decision makers. The NP must comply with the nursing practice standards used by the College of Nurses of Ontario.

Roles and Responsibilities:

  • Provides most responsible clinical care to an established co-hort of patients assigned to the Nurse Practitioner
  • Provides acute and episodic care to all patients of the Family Health Team as required
  • Conducts periodic examinations and risk assessments that may include care of at-risk populations, wellness exams, pap tests, breast exams, pediatric care, well-baby visits and immunizations
  • Monitors the ongoing therapy of patients with chronic stable illness by providing effective pharmacological, complementary or counseling interventions
  • Undertakes patient home visits within the catchment area, as needed
  • Maintains accurate, concise and confidential EMR documentation pertaining to patient history and encounter data
  • Recommends and arranges for specialist referrals and appointments in consultation with physician(s)
  • Collaborates with other multidisciplinary team members and external partners
  • Develops and conduct a broad range of health promotion and chronic disease management processes, programs, and patient education (group and individual) in collaboration with multidisciplinary team members
  • Participates in ongoing quality monitoring and improvement
  • Communicates ideas and concerns to the team and participate in problem-solving
  • Participates in educational sessions held by other multidisciplinary team members
  • Teaches and mentor team members on the role of the Nurse Practitioner.

Interprofessional Collaboration:

  • Develops and hones collaborative community relationships to facilitate patient access to services upon physician/health care team request;
  • Communicates ideas or concerns to the health care team. Participate in problem solving to enhance patient care;
  • Assists in the training and development of new staff.

Practice Environment:

  • Use of personal protective equipment with all patients;
  • Ensures appropriate storage and expiry date of immunizations;
  • Ensures emergency medications (e.g. epinephrine or Benadryl) are current and accessible;
  • Assists with other related duties as required.

Education/Experience:

  • Current registration with the College of Nurses of Ontario as a Nurse Practitioner – Primary Health Care
  • Two– five years’ experience providing comprehensive primary care in a team-based setting.

Skills/Abilities:

  • Demonstrated ability in utilizing the nursing process in planning, implementing and evaluating patient care;
  • Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery, with an emphasis on health promotion, risk reduction and chronic disease management;
  • Leadership, organization, research, evaluation, time management, communication and interpersonal skills;
  • Demonstrated ability in problem solving using sound judgment and critical thinking;
  • Strong organizational, interpersonal and communication skills;
  • Ability to prioritize, manage time effectively and be flexible in a very active constantly changing work environment which includes being flexible to work daytime and evening hours;
  • Proficiency in the use of the computers and information technology is required including the use of electronic medical records (EMR).
  • Excellent health assessment and clinical skills
  • CPR certification
  • Excellent verbal and written communication skills
  • Ability to work collaboratively as a member of the FHT’s multidisciplinary team.
  • Current Ontario Driver’s License
  • Ability to work flexible hours
  • Health promotion skills, including population health and stress management, an asset.
