CASE MANAGER - REGISTERED NURSE (RN)

We are seeking a dynamic and innovative individual who will work enthusiastically to deliver recovery oriented, client and family centered and comprehensive services that meet client needs, ensures client safety and works within a quality improvement framework to identify opportunities for enhancing quality and quantitative outcomes of the services. Client and family centered care is guiding principle at Cota that helps improve decision making processes, health outcome and client experiences. The successful individual will embed this guiding principle into their work.
About the Position:
Cota seeks a Case Manager-Registered Nurse for our Scarborough team. The mental health nurse will be the newest member of our Scarborough Team. This intensive case management team is one of 4 that are multidisciplinary, with case manager specialists working with diverse populations including serious mental illness, addictions, acquired brain injury, geriatric mental health conditions and dual diagnosis.
As a member of this team, you will have an independent case load as well as being responsible to provide expertise in the areas of physical and mental health assessments and metabolic monitoring. The Case Manager- RN is also responsible for providing oversight, consultation, education and administration of medication.
SUCCESSFUL CANDIDATE REQUIRES:
- A Baccalaureate degree in nursing from a recognized university
- The ideal candidate will have 2 years direct experience in community mental health nursing, however new grads are welcome to apply
- Maintains a certificate of competence with the College of Nurses of Ontario
You must also have:
- Sound clinical knowledge and experience in case management with the client population, including individuals experiencing serious mental health issues and homelessness
- Demonstrated strong commitment to the right and ability of each person with a serious mental illness to live in normal community residences, work in market jobs, and have access to helpful, adequate, competent, and continuous supports and services
Demonstrated ability to
- Conduct home visits and to exercise judgement with a high degree of autonomy
- Establish supportive, trusting relationships with persons with a serious mental illness with respect for clients’ rights and preferences
- Use problem-solving techniques to identify and assess client need and develop appropriate intervention strategies
- Assess mental health needs, physical health needs, functional capabilities and care options
- Ability to make appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric and medical treatment
- Offer groups and individual services to educate clients, families and other staff about health and wellness behaviours (e.g., diabetes management)
- Handle ambiguity to function effectively while dealing with competing priorities
- Familiarity with the Ontario Mental Health Act and Mental Health Reform
- Knowledge of Recovery as a guiding principle for assessing and developing treatment plans for clients
- Ability to work with agencies, hospitals and the broader community
- Knowledge of trends and issues related to the delivery of services to a mental health client population
- Excellent organizational and interpersonal skills
- Excellent computer skills (e.g., Word, Excel, Microsoft Outlook)
- Ability to manage and adapt positively to an ever changing health care/work environment
- Effective oral and written skills
- Experience working in a community agency an asset
- Reviewing the referral information of applicants to the program
- Determine eligibility for the program and confirm acceptance of new clients into the program
- Developing a collaborative, supportive and therapeutic relationship
- Completing a comprehensive psychiatric and physical health assessment of the nature of each client’s unique goals and needs through collaborative engagement with each client and their respective support networks
- Providing guidance and assistance to each client in relation to the development of both a personalized Client Safety Plan and a Recovery/Goal Plan
- Provides health education to clients and team to promote health and well being
- Provide direct practical support, coaching and counsel to clients in their pursuit of identified goals and needs.
- Assisting clients with emergency and/or crisis needs as required/appropriate
Documentation and Data entry
- Documents all client contacts/interactions in the appropriate client chart according to Cota’s documentation guidelines and College standards
- Assists with other data collection activities within the program, as determined by the Program Manager
- Participates in team meetings, provides input and ideas for improving services, efficiencies and ongoing program planning
- Participates in any research-related activities
- Visits will be conducted in the clients’ homes, hostels, hospitals and other community settings.
- Weekend and evening rotation may be required.
- Be required to have a valid driver’s license and a reliable vehicle.
