Registered Nurse, East End Community Health Centre

Hours of work:  Full-time: 5 days per week


As a member of an interdisciplinary team, the registered nurse functions interdependently with other Centre staff to provide shared, client-centered care that emphasizes health promotion, disease prevention and client education. The registered nurse provides primary health care within the full scope of practice to clients at all stages of their lives. This position shares care with physicians and nurse practitioners for clients who have complex and/or chronic health conditions. The registered nurse practices according to the Standards of Practice of the College of Nurses of Ontario (CNO) and to guidelines and policies established at East End Community Health Centre.


  • Ensures that standards of client care are maintained according to accepted College of Nurses standards, the Regulated Health Professions Act, and Centre policies
  • Assumes primary care nursing role in the areas of acute and chronic disease prevention and management
  • Monitors clients with episodic and stable chronic conditions commonly seen in primary health care, consulting with CHC physicians and NPs when issues present that go beyond the RN scope of practice
  • Performs telephone triage function to clients as part of the primary care team
  • Performs venipuncture, ECGs, and other tests when clinical assistants are not available and as needed
  • Makes internal referrals to other East End CHC staff as appropriate, and refers clients to external specialists, hospitals or community resources as appropriate
  • Responsible for the clinical assessment of new clients to the Centre
  • Assists with the coordination and implementation of quality improvement activities across the clinic
  • Integrates health promotion in the realms of lifestyle and nutrition into each interaction with individuals, families or groups through education and counselling



Education and Professional Requirements

  • Current registration as a Registered Nurse with the College of Nurses of Ontario, and certificate of continuing competence
  • BSc in Nursing from a recognized university or equivalent education and experience
  • Certified Diabetes Educator training is an asset

Knowledge and Skills

  • 3-5 years of recent experience in a primary care role in a community setting or a combination of community experience and relevant hospital or public health nursing
  • Knowledge and experience of primary care issues and challenges for high risk communities
  • Maintains knowledge of current social/health issues, and legislation affecting the practice of nursing
  • Comprehensive knowledge of applicable occupational health and safety legislation
  • Demonstrated ability to work effectively in a multi-disciplinary environment
  • Proficiency in the use of computer technology and various software applications
  • Strong oral and written communication skills
