Cota is an accredited community-based organization that has been supporting adults with mental health and cognitive challenges to live well within their communities for over 40 years. We provide person-centred supports that assist individuals to find inspiration and hope in their inherent strengths and support them to pursue meaningful change in their lives, as they define it. We also work collaboratively with our funders, our partners and other stakeholders, across the provincial health and social service systems, to inspire positive change in the way that services are delivered to individuals living with mental health and cognitive challenges.

We are seeking a dynamic and innovative leader who will work enthusiastically to support the delivery of recovery oriented, client and family centered and comprehensive services that meet client needs, ensures client safety and works within a quality improvement framework to identify opportunities for enhancing quality and quantitative outcomes of the services.  Client and family centered care is guiding principle at Cota that helps improve decision making processes, health outcome and client experiences. The successful leader will oversee their portfolio in accordance with this guiding principle.

The Program Manager of Community Health Services is accountable for providing leadership in the development and delivery of high quality recovery oriented and client centred services at Cota.  This position is responsible for a portfolio which includes but is not limited to:

  • “At Home” Assertive Community Treatment (ACT) Team
  • “FOCUS” Integrated Service Coordination Team

The “At Home” ACT Team participated in the Mental Health Commission of Canada’s national research demonstration project on Housing First and continues to serve homeless individuals with serious mental illness and substance abuse issues. The Program Manager is responsible for the daily operations and clinical supervision of this multidisciplinary team, ensuring the program maintains fidelity standards for the ACT model and supports ongoing research protocols. Cota’s ACT team provides housing opportunities for clients using a “housing first” scattered site approach. Clients may also receive access to an At Home rent subsidy without requiring participation in psychiatric or substance use treatment. Self-determination, choice and harm reduction are at the centre of all considerations with respect to housing and support services.

The “FOCUS” Team involves a partnership between Cota and St. Michael’s Hospital to deliver an innovative Integrated Service Coordination (ISCT) Model. The Cota Program Manager works closely with the St Michael’s Hospital clinical leadership team to co-manage this program. The Cota Program Manager recruits and supervises 8 Cota staff to work seamlessly within this integrated multidisciplinary partnership team serving complex populations within the Mid-East Toronto Health Link.  St. Michael’s Hospital maintains primary responsibility for daily team operations. The FOCUS team draws heavily on elements of the Flexible Assertive Community Treatment (FACT) model offering a flexible approach to deploy two levels of support (assertive community treatment and intensive case management).

Summary of Duties:

  • Hire, train, supervise and provide administrative and clinical support to frontline staff
  • Establish new and maintain ongoing positive relationships with partner agencies and external stakeholders
  • Identify and implement quality improvement initiatives as required
  • Oversee management of program budgets with internal support
  • Play a lead role in developing/maintaining and providing education of program specific operating standards / guideline manuals
  • Act as a representative of Cota at various network and/or external committee meetings
  • Participate as a member of the Clinical Leadership Team in planning / developing of the team’s goals and objectives as well as participating in a number of organization-wide activities
  • Act as a member of the Cota Leadership Team
  • Report directly to Director, Community Health Services

Summary of Qualifications:

  • Graduate of a recognized University Program in a related health care or social service discipline; a Nursing degree will be considered an asset
  • Strong clinical skills with at least 5 years’ experience providing community-based treatment and services to urban homeless populations with serious mental illness, substance addiction and complex health conditions
  • One year supervisory or program management experience with a multidisciplinary team. Previous management experience of an ACT team will be considered an asset
  • Demonstrated excellence in organizational, interpersonal and communication skills, team building and coaching abilities are essential
  • Experience in hiring, training, supervising and retaining qualified staff
  • Knowledge of the Housing First, FACT and ACT models and the Ontario ACT Program Standards required
  • Strong knowledge of evidence-based practices used in the mental health and addiction field including recovery practices and harm reduction
  • Solid team player with a demonstrated ability to work with flexibility and efficiency in a fast-paced environment
  • Ability to work effectively with individuals with complex needs and multi-system involvement
  • Ability to maintain a positive, solution-focused approach when responding to conflicts or problems
  • Ability to go above and beyond and demonstrate initiative
  • Experience understanding and managing fiscal aspects of the program
  • Proven cultural competence and ability to work effectively with diversity
  • Knowledge of information management, quality improvement and evaluation activities
  • Experience in networking and working in partnership with other groups/agencies
  • Current Registration in good standing with the respective professional college (e.g., OT, SW, RN, etc.) will be considered an asset
  • Experience working in a unionized environment in a supervisory capacity will be considered an asset
  • Familiarity with Anzer electronic health record, OCAN and OPOC reporting  will be considered and asset
  • Working knowledge in Microsoft Office – Word, PowerPoint, Excel, Outlook

 Work conditions:

  • Generally Monday through Friday
  • Ability to work occasional week ends / evening hours
  • Rotating on-call responsibilities

Salary Range: $72,000-$76,000 annual salary
