Job Title: Nurse Practitioner 

Main Purpose of Job:  Provide comprehensive primary care to patients under the direction of Medical Director. 

Summary: 

Employee will provide comprehensive primary care by assessing, examining, investigating, diagnosing, treating, and counselling patients in a collaborative, interdisciplinary team of allied health professionals to deliver a patient-centred model of care. They will, at times, operate under the delegation of a physician while at other times operate as an independent care provider under their Nurse Practitioner license. Employee will function in a clinic setting with a focus on health promotion, disease prevention, chronic disease management and acute illness management as they respond to the needs of our local community. 

Duties and Responsibilities

Direct Patient Care 

  • Provide patient-centred care planning with interdisciplinary team to provide high-quality, evidence-based care to the patients 
  • Provide comprehensive primary care by assessing, examining, investigating, diagnosing, treating, and counselling patients 
  • Monitor the ongoing therapy of patients with chronic stable illness by providing effective pharmacological, non-pharmacological, and counselling interventions 
  • Safely prescribe medication while ensuring patients are informed of benefits and risks associated with pharmaceutical therapy 
  • Provide care with a focus on health promotion, disease prevention, chronic disease management, and acute illness management to patients with various illnesses, diseases, and impairments 
  • Communicate directly with patients and provide medical direction 
  • Ensure patient preventative care guidelines are followed 
  • Administer and remain current on guidelines for immunizations 
  • Assess vital signs including temperature, heart rate, blood pressure, oxygen saturation monitoring, and glucometer testing 
  • Complete patient care flow sheets 
  • Administer medications 
  • Document on electronic medical record 
  • Assist with, and perform once trained, procedures, including join injections, excisions, and cryotherapy

    Education: 
  • Provide health education and health promotion information to individuals and groups 
  • Supervise and support the educational development of students from related disciplines

    Collaboration: 
  • Consult with physician in accordance with the standards for consultation with physicians, and/or refer the patient to another health care professional 

Primary Care Model Developmental Work:

  • Develops evaluation strategies with the primary care clinic around the effectiveness of the chronic disease management and prevention program 
  • Assists in planning and organizing objectives and long-range plans for primary care clinic operation in relation to chronic disease management and prevention program and the maternal/child care program 
  • Works with other members of the Primary Care team to develop and implement research projects 

Community/Networking:

  • Develops strong network of community resources 
  • Referral of patients to appropriate community services 

Qualifications: 

  • Baccalaureate Degree in Nursing required, Masters of Nursing or Health related field preferred 
  • Primary Health Care Nurse Practitioner Certificate required 
  • Current registration with the College of Nurses of Ontario in the Extended Class with no
    restrictions required 
  • Must carry professional liability insurance to practice as a NP at the NPs expense 
  • Mandatory COVID-19 vaccination

Knowledge and Skills: 

  • Two to three years clinical experience as a Nurse Practitioner preferably within a multi- disciplinary family practice 
  • Advanced decision making, problem identification and problem-solving skills 
  • Excellent interpersonal and communication skills (both verbal and written) 
  • Ability to resolve conflicts and assist with the management of distressed patients 
  • Excellent organizational and time management skills 
  • Ability to work as part of a team in a health care environment 
  • Proficient in the use of computer and information technology 
  • Experience working with an Electronic Medical Record (preferred) 
  • Ability to multi-task and work efficiently and safely 
  • Positive attitude and emphasis on patient care 
  • Demonstrates confidentiality

    Physical Requirements:
    Given that the employee will be directly working with patients suffering from multiple illnesses, diseases, and impairments, specific physical requirements are required to ensure patient and employee safety.
    Employee must be sufficiently mobile to rapidly move between workstations within the office. Employee must be able to assist patients in moving between waiting room and examination room. Employee must be able to assist patients in transferring between sitting and standing, between standing to sitting on examination table, and between sitting to lying down on examination table. Employee must be able to physically examine patients requiring leaning over patients, bending down, and lifting up to 30 lbs.
