At Surrey Place we help children and adults living with developmental disabilities, autism spectrum disorder and visual impairments reach their full potential. We offer a variety of groups and workshops for clients, families and caregivers, as well as extensive education and consultation services to community agencies.

Our comprehensive programs and services range from assessment, diagnosis, and one-on-one treatment, to family counselling and group support and is provided by a broad network of clinicians and professionals. Surrey Place is accredited with Accreditation Canada and is affiliated with the University of Toronto and other academic institutions and is a teaching site for students in a variety of health care professions.   

Surrey Place employs nearly 500 employees, with 12 locations in the GTA and we also provide video-conferencing services to communities in northwestern Ontario.

Position Description:
The Registered Nurse (RN) practices as member of the primary care team, providing health care to Surrey Place clients. In this role, the RN will emphasize health promotion, health education and disease prevention. Reporting to the Senior Director, Children’s Services, the RN contributes to excellence of care for infant and child aged clients and families coping with developmental disabilities and contributes to the development of models of health care for these clients.

Main Responsibilities:

  • Conduct comprehensive health assessments of infants, children, and transition aged youth with developmental disabilities.
  • Formulate, implement, and evaluate plan of care, involving clients and/or care givers for health needs appropriate to RN scope of practice, and specifically focus on developmental disabilities using evidenced-based care.
  • Contribute to the management of episodic and stable chronic conditions commonly seen in primary health care, consulting with Surrey Place physicians, Nurse Practitioners, and/or Clinical Nurse Specialists.
  • Work with clients, families, internal and external inter-professional teams to navigate the health care system and identify and access health and other resources in their communities.
  • Act as a Case Manager to help clients and families access resources and help coordinate care of complex clients with the Surrey Place team and community partners.
  • Support the TRE-ADD program and BMACKE Clinic as part of an interdisciplinary team, providing longitudinal physical and mental health care to complex youth including:
    • Intake, triage and pre-screening of all new referrals;
    • Conduct comprehensive physical/mental health assessments of youth with developmental disabilities;
    • Liaise with community health providers to ensure continuity of care;
    • Nursing assessment and management of chronic/episodic physical/mental health concerns for youth within the classroom;
  • Integrate health promotion principles into each interaction with individuals, families or groups.
  • Participate in community projects or agency activities as a representative of Surrey Place as directed.
  • In collaboration with medical staff, builds community capacity within the nursing/medical community to provide services for people with developmental disabilities.
  • Participates in developing and implementing IPAC policies in response to the COVID-19 pandemic.

Qualifications:  

  • Current registration as a Registered Nurse in the General Class with the College of Nurses of Ontario, and certificate of continuing competence.
  • Baccalaureate degree in nursing from a recognized university or equivalent education and experience.
  • Three years of nursing practice experience in a community setting or a combination of community experience and public health nursing is essential. Relevant hospital nursing an asset.
  • Experience working with individuals with developmental disabilities, an asset.
  • Experience with case management an asset.
  • Thorough current nursing knowledge and proficiency in conducting a comprehensive health history, physical assessment and construction of a nursing management plan.
  • Demonstrated ability to work effectively in a multi-disciplinary environment.
  • Demonstrated commitment to and knowledge of community-based health care.
  • Good communication and interpersonal skills, with a demonstrated ability to work well with others.
  • Second language fluency in French or a language relevant to the Surrey Place Community in addition to English.
  • The successful candidate must provide an acceptable Vulnerable Sector Screening, and proof of being fully vaccinated for Covid-19.
  • Proficient with Microsoft Office.
