Brock Community Health Centre (Brock CHC), a charitable non-profit community health centre serving the residents of Brock Township and surrounding area, is committed to the provision of quality primary health care services, health promotion, education and illness prevention. Funded by the Central East Local Health Integration Network and the Ontario Ministry of Health and Long-Term Care, Brock CHC collaborates with community partners to provide a range of health services to residents with a focus on the social determinants of health.

If you are a Registered Practical Nurse who is determined to make a difference, this is an opportunity for you. As a member of the Brock Geriatric Assessment Program, you will partner with the Nurse Practitioner in the provision of specialized geriatric services to frail elderly residents of Brock Township and surrounding areas (.5), and enhance services for Ontarians with behaviours associated with complex and challenging mental health, dementia or other neurological conditions (.5).

Key Responsibilities:

• Assist with implementing comprehensive assessment of seniors living at home or in retirement residences with complex health care needs

• Assist in development of comprehensive care plans, including evaluation and modifications to ensure effective care

• Conduct behavioural assessment for clients, from a holistic lens, including client interviews and continuous BSO support visits

• Conduct associated screenings such as Confusion Assessment Method (CAM), Mini-Mental State Examination (MMSE), Geriatric Depression Scale, Montreal Cognitive Assessment (MoCA)

• Assist in leading/supporting BSO quality improvement initiatives

• Collaborate with other health and social services to maintain or improve health; promote functional independence and quality of life; and prevent or delay institutionalization as appropriate

• Assist in the education to caregivers/family members on awareness and care for geriatric syndromes

• Document client care in the EMR according to organizational policies and College of Nurses standards of practice

• Actively participate in an interdisciplinary team environment within the CHC model of care

Key Qualifications:

• Current registration in good standing with the College of Nurses of Ontario

• Three years recent experience as an RPN working in geriatric and long-term care

• Demonstrated extensive knowledge and skill working with individuals with various types of dementia, delirium, mental health, addictions, other neurological conditions and those who have experienced acute behavioural change

• Experience documenting in an Electronic Medical Record (PS Suites considered an asset)

• Training or the ability to obtain training in P.I.E.C.E.S., Gentle Persuasive Approach, U-First! and Montessori

Remuneration: $53,054 to $64,056 annually (based on experience). This role will require some evening work and local travel.
