Inner City Health Associates (ICHA) is group of more than 100 physicians working in over 50 shelters and drop-ins across Toronto. ICHA provides transitional primary care and specialist (i.e. psychiatry, substance use, and palliative care) to those who do not otherwise have access to care. We serve people who are unsheltered and temporarily living in shelters as well as those who are precariously housed. ICHA is very pleased to be expanding its interprofessional team to include nursing to offer enhanced client supports. Many of ICHA’s clients have complex health needs including untreated chronic and complex multi-system diseases, an elevated risk of acute health emergencies, and serious and persistent mental illness and substance use.
ICHA is providing clinical support to facilities for homeless individuals who are isolating due to a confirmed or potential COVID-19 diagnosis. Nurses will use their knowledge, skill and judgment to assess and care for clients who may present with complex health issues in addition to COVID19 such as unmanaged chronic conditions, infectious diseases, skin conditions and wounds, mental illness and substance use issues. Services provided by the nursing team will use a holistic, client-centred model of care with an emphasis on trauma-informed primary care, harm reduction and conflict de-escalation.
The ICHA RN and RPN collaborates with ICHA Physicians, peer support workers and/or case managers and shelter staff. They will complete the intake assessment in collaboration with the ICHA on-call physician and nursing team. They will provide ongoing health assessment of clients’ physical and mental health through regular wellness checks on stable clients and more frequent assessment of clients with more complex medical or social needs. The site is managed by City of Toronto staff with community partners. Nurses will collaborate with city/shelter staff and peer support workers to provide social and emotional supports including crisis mitigation. There will be an ICHA Nurse Manager and/or Charge Nurse providing supervision and support, both on site and via phone. The nurses will work under the medical direction of an ICHA Medical Director and RN Director of Clinical Services.
- Provides nursing care and treatment through pharmacological, and/or counseling interventions to a marginalized individuals presenting with complex health needs. These may include wounds, skin disorders, infectious diseases and mental health and substance use issues.
- Performs individual client intake assessments (physical, emotional and psychological in collaboration with ICHA MD and nurses including client history, recording of baseline vitals, and risk assessment.
- Provides ongoing assessments of patients with acute respiratory symptoms and monitoring for signs of respiratory distress, and the signs and symptoms of COVID-related deterioration. In collaboration with MD, nursing team and shelter/city staff, RN/RPN will identify patients in need of transfer to hospital.
- Provide ongoing mental health assessment and support in collaboration with peer support workers, community health workers and ICHA nursing team and MDs.
- Dispenses and administers medication/drugs as prescribed to treat symptoms, diseases or disorders within the RN/RPN’s scope of practice, including over-the-counter (OTC) and PRN medications
- Collaborates with clients, physicians and other health members to develop appropriate, client-centred, individualized care plans.
- Engages clients in risk factor and health problem identification, goal setting and making choices for intervention in disease treatment, disease prevention and health promotion.
- Keeps complete and accurate records of client interventions and care through timely documentation in the Inner City Health Associates Medical record (EMR—OSCAR) as per CNO Standards
- Actively participates in clinical huddles, team rounds, case conferences, staff meetings and other clinical education opportunities.
- Provides diligent and conscientious care in accordance with IPAC requirements.
- Complete all online mandatory training prior to start date and OPS training within 3 weeks after the start date.
QUALIFICATIONS, SKILLS and EXPERIENCE
- Current Registration and member in good standing with the College of Nurses of Ontario with no restrictions
- Current CPR certification
- Excellent verbal and written communication skills
- Police check/clearance documentation (i.e. vulnerable sector screen) required
- Minimum 1 year of clinical experience after completion of degree/diploma
- Knowledge and experienced in the application of electronic medical records (EMR)
- Knowledge and a demonstrated ability to provide nursing care in community health, primary care and public health settings or equivalent relevant expertise
- Experience working with clients who use substances and/or who have serious and persistent mental illness
- Willingness to work from a harm-reduction and anti-oppression, trauma-informed framework; experience working with homeless or marginalized service users preferred
- Knowledge of and commitment to client–centered philosophy
- Open minded, non-judgmental with a commitment to upholding the rights and dignity of all people and respecting their diversity
- Knowledge and proficiency in current, evidenced-based methods and practices in health care delivery, with an emphasis on mental health, health promotion and risk management/reduction
- A sound understanding of and commitment to the social determinants of health, and the ability to effectively participate in the efforts of an interdisciplinary team to contribute to comprehensive health care
Requirements specific to Registered Nurses (RN)
- A Registered Nurse with baccalaureate degree in nursing or equivalent
- Current RNAO membership and evidence of liability insurance
Requirements specific to Registered Practical Nurses (RPN)
- A Practical Nursing Diploma or equivalent
- Current WeRPN membership and evidence of liability insurance
