Accountability

Under the general supervision of the Health Director and clinical supervision by the Zone Nursing Officer, Moose Factory Zone, First Nation Inuit Health, (FNIH) Indigenous Services Canada (ISC), the Community Health Nurse (CHN) is responsible for the delivery of the community health program. These programs and services shall be acknowledged and supported by the Peetabeck Health Services Board and supported by Chief and Council.

Position Summary

The CHN shall assist with the planning, organizing, implementing and evaluation of a comprehensive community health program for the health and wellness throughout the community. To act as a supervisor to the Community Health Department staff to provide supervision and direction for program planning, to ensure compliance with the Human Resource Policy and to ensure that month end and other reports are submitted.

Major Responsibilities

1. Establish an on-going planning of community health programs and education, health promotion and prevention as per First Nations and Inuit Health Branch, Indigenous Services Canada guidelines for mandatory and non-mandatory community health programs.

2. Establish program priorities in respect of community health needs.

3. Provide prenatal guidance and support. Develop and implement community based pre-natal classes. Monitor high-risk pregnancies and provide home visits for follow up on all pre-natal clients.

4.Coordinate and implement Well Baby Clinics with physical examinations, monitor growth and development, provide support to mothers and administer immunizations as per the provincial guidelines for immunizations.

5. Conduct home visits to identified high risk clients and offer support and guidance.

6. Communicable disease follow ups by providing contact tracing, dispensing antibiotics and reporting as per FNIH guidelines.

7. Implement Tuberculosis Program and protocol as per FNIH, ISC. Consult with CDC, FNIH, ISC as per protocol.

8. Make regular school visits and act as resource to the teachers and to provide school health education as per the needs identified by the school educators.

9. Maintain records of persons with Chronic disease ensuring follow up with home visits.

10. Provide guidance and direction to the community health team for community health programming.

11. Plan and implement community health sessions for across the life span.

12. Documenting clients charts as per FNIH, ISC documentation standards

13. Organizing and supervising equipment and facilities where appropriate.

14. Prepare monthly reports and plans for the Health Director.

15. Monitoring medications, vaccines and ensure supplies ae stocked according to the established policy.

16. Meet with the visiting physicians, optometrists, dentists, hospital authorities and other organizations involved in the delivery of community health to the community members.

17. The Zone Nursing Officer, FNIH, ISC, shall provide clinical support and supervision of the CHN as it relates to nursing and clinical responsibilities.

18. Attend meetings and workshops to enhance professional growth as required.

19. To provide occasional nursing relief and nursing consultation as needed for the other programs that require nursing skills and knowledge.

20. To act as a Supervisor to the Community Health Team staff and have monthly team meetings to plan a monthly community health program calendar.

21. To ensure prompt communication with the community as it relates to drinking water alerts.

22. Other duties as assigned.

Qualifications

1. A valid license to practice nursing in the province of Ontario with a CNO designation of Entitled to Practice.

2. Graduation with a degree in nursing from a recognized post-secondary institution.

3. Recent (within last 5 years) experience as a Registered Nurse in the area of community health nursing including Primary Health Care.

4. Experience in providing health care services to indigenous People and/or Indigenous communities.

5. Experience in managing human resources.

6. Experience working with or building partnerships with internal and external stakeholders.

7. Experience in providing presentations and/or preparing reports.

8. Experience in working in public health units.

9. Willingness and ability to work overtime as required.

10. Willingness to travel on gravel roads and varied weather condition to work in a remote and isolated community.

11. Willingness and ability to complete mandatory education/training relevant to the position.

12. Knowledge of principles, practices of professional nursing and to incorporate to the community environment.

13.Ability to plan, prioritize, implement and evaluate programs.

14. Must possess a valid Ontario Driver's license.

15. The ability to speak Cree is considered a definite asset.

16. Must provide proof of liability coverage through RNAO.
