The below noted tasks are to be completed in a manner which builds the business and meets the strategic needs of the organization.

PRIMARY DUTIES AND RESPONSIBILITIES

Advances Integrated Care, Standards of Care and Professional Practice Leadership

  • Accountable for implementation at a local level and collaborate at regional level of clinical standards, educational programs and related clinical orientation and training
  • Provide independent high-quality care to a limited number of patients with complex needs 
  • Champions integrated patient-centred seamless care, at a local and regional level, through the use of best practices, common documentation, integrated care coordination and other collaborative approaches

Build Capacity and Knowledge Mobilization

  • Work closely with Professional Practice Advisor (PPA) to help inform clinical design at local and regional tables with internal and external stakeholders 
  • Mentoring, coaching others at a local level and collaborate with other Clinical Practice Leads and PPAs at regional level
  • Act as clinical preceptor who coaches and mentors nurses, personal support workers or therapists, as needed
  • Provide input and participate in continuous quality improvement initiatives (process improvements, efficiency, etc.)
  • Work with local and regional management on clinical goal setting and performance management
  • Supports visiting nurses during their ‘on call’ coverage, as needed during regular hours, after hours or weekends
  • Enhance local capacity and understanding of system navigation supports, at a local and regional level, to support holistic patient care delivery 
  • Responsible for the delivery of education designed by the Professional Practice, Research and Education team 
  • Coach integrated teams in the implementation of new clinical programs for specific patient populations as identified by Ontario Health Teams (e.g. COPD, CHF, etc.)
  • Uses new technology and supports innovative models of integrated care

Research & Policy

  • Liaises with Professional Practice Advisor(s) and Operations managers and/or Director on ongoing best practices, learning opportunities to inform policy and research 

Special projects 

  • Work on projects focused on high quality patient centred integrated care and advancing clinical practice across the organization. 

Reporting Structure

  • Reports to one Professional Practice Advisor (PPA), however connects with other PPA based on specialty role and local needs
  • Dotted line of reporting to local Operations Manager to ensure that the position is supporting local branch requirements
  • Work closely with local Nurse Managers and Personal Support Managers (PSMs)

OCCUPATIONAL HEALTH AND SAFETY RESPONSIBILITIES

  • Use or wear the equipment, protective devices or clothing that the Employer requires to be used
  • Report to the supervisor safety hazards and not proceed with the unsafe activity until safe alternatives are provided by the supervisor / manager
  • Attend safety training(s) as required
  • Apply safety techniques and processes as trained
  • Participate in Joint Health and Safety Committee’s undertakings
  • Request further information and training if safety training is unclear or confusing
  • Work in compliance with the Occupational Health and Safety Act, legislation and best practices

QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • Regulated Health Professional (for e.g., RN, RPN, OT, PT) 
  • More than 2 years of community experience in leading exceptional clinical practice in a community care environment; a combination of experience and education will be considered
  • Proven clinical experience in providing high quality clinical care, strong assessment skills, critical thinking and problem solving
  • Proven experience leading and mentoring people delivering community care
  • Knowledge of adult learning principles, as asset
  • Strong proficiency in all aspects of Microsoft office;
  • Keen attention to detail, ability to prioritize, multitasking skills and organizational skills;
  • Strong written communication, proofreading and editing skills required to draft and/or generate correspondence/documents;
  • Ability to multi-task with an attention to detail, demonstrating organizational, prioritization, and time-management skills, as well as proven ability to meet deadlines
  • Strong analytical skills;
  • Demonstrated ability to handle highly sensitive and confidential information.
  • Ability to travel within Ontario on an occasional basis
