Job Title: Registered Nurse (RN) Coordinator
We Count COVID-19 Indigenous Testing Site
Salary: $50.00/hour, variable shifts (days and evenings, 5-7 day per week)
Hours: 0ne position, starting at 0.5 FTE (17.5 hours) progressing to 1.0 FTE (35 hours)
Location: 22 Vaughan Road

Inner City Health Associates (ICHA) is group of more than 100 physicians working in over 50 shelters and drop-ins across Toronto. ICHA provides transitional primary care and specialist support (i.e. psychiatry, substance use, and palliative care) to those who do not otherwise have access to care. We serve people who are unsheltered and temporarily living in shelters as well as those who are precariously housed. ICHA is very pleased to be expanding its interprofessional team to support the We Count COVID-19 Indigenous Testing Site which is housed at Native Men’s Residence (NaMeRes) and additionally supported by Seventh Generation Midwives Toronto, Well Living House (Unity Health Toronto) and the Centre for Wise Practices (Women’s College Hospital). The We Count COVID-19 (WCC) project uses Indigenous community-driven processes to address gaps in health and public health services and information for First Nations, Inuit, and Métis peoples impacted by COVID-19. The project includes building a culturally safe and Indigenous-specific Covid-19 testing site that provides COVID-19 screening with integrated social support and public health follow up.

Role Description:
Reporting to the ICHA Director of Clinical Services and working closely with the We Count COVID-19 Clinical and Public Health Lead and NaMeRes Assessment Site Lead, and the rest of the WCC team, the We Count COVID-19 Indigenous testing site RN Coordinator will support holistic care for adults and children attending the WCC Indigenous COVID-19 testing site. This will include providing direct wrap around supports to clients, including assessment, management of and referral for urgent unmet health and social needs; COVID-19 screening and risk factor assessment; client navigation of the testing site in a manner that optimizing infection prevention and control (IPAC); administration of COVID-19 testing; follow-up on COVID-19 testing results; client education regarding COVID-19 prevention; and support for appropriate case management. In addition, working with WCC-ICHA nurse practitioner and clinical/public health lead, the WCC assessment site RN Coordinator will co-lead the management of the
testing site, including support to ensure the day to day adequacy of equipment and supplies and ongoing implementation of IPAC measures.

Roles and Responsibilities:

- Screen clients presenting for COVID-19 testing in a culturally safe manner

- Obtain Covid-19 test (i.e. under direct client order or medical directive) and collect specimen according to public health, clinical and IPAC guidelines and standards

- Provide tailored COVID-19 prevention and management counselling

- Review and communicate Covid-19 results in a timely manner and arrange appropriate follow up in collaboration We Count Covid-19 outreach workers and Toronto Public Health

- Consistently document in the clients' electronic medical record following College of Nurses of Ontario Standards of Practice for Documentation

- For participants who consent to participation in the We Count Covid-19 database, gather and record registry information

- Maintain accurate clinical and registry records

- Assess and address any urgent unmet clinical and social needs that arise on-site during opening hours

- Provide appropriate disposition management in collaboration with the We Count Covid-19 outreach workers and other COVID-19 community supports, including referral to Covid-19 recovery sites for persons experiencing homelessness

- Collaborate with other Indigenous and allied service organizations to develop and implement culturally safe defined referral pathways

- Work in collaboration with other team members and Indigenous and allied service organizations as required to ensure a smooth transition between care sites which meets the client's needs

- Coordinate medical supplies procurement in collaboration with other site staff

- Participate in staff meetings and contribute to site quality improvement and review of procedures and policies.

- Perform other related duties as assigned by the testing site and WCC project leadership

- A Registered Nurse with baccalaureate degree in nursing or equivalent

- Current Registration and member in good standing with the College of Nurses of Ontario as a Registered Nurse with no restrictions

- Current RNAO membership; evidence of liability insurance

- Demonstrated knowledge and experience engaging in and working with Indigenous communities

- Ability to contribute to a culturally respectful and inclusive Indigenous space

- Knowledge and a demonstrated ability to provide nursing care in community, primary care and/or public health settings or equivalent relevant expertise

- Able to work independently in non-typical community-based primary care setting with remote MD support as needed

- Knowledge of relevant community services for individuals experiencing homelessness

- Demonstrated thorough knowledge and understanding of infection control and prevention practices

- Experience with trauma-informed care; able to communicate with clients in a professional, empathetic and culturally safe manner

- Strong communication, team-oriented interpersonal skills and organizational skills

- Flexibility to adapt to a dynamic and changing situation and workload

- Current CPR certification

- Knowledge and experienced in the application of electronic medical records (EMR)

- Experience working with Covid-19 patients is considered an asset

- Experience working in clinical research is also considered an asset
